NRG1 combined with Total MRI Burden on Cerebral Small Vessel Cognitive Dysfunction of the Predicted Value
Objective To analyze and explore the predictive value of Neuregulin 1(NRG1)and the total load score of Magnetic Resonance Imaging(MRI)for cognitive impairment in cerebral small vessel disease.Methods A retrospective analysis study was conducted on 124 high-risk individuals with cerebral small vessel disease(CSVD)and inpatients with vascular cognitive impairment(VCI)who were admitted to the Second Affiliated Hospital of Henan University of Science and Technology from October 2021 to October 2022.According to the Montreal Cognitive Assessment(MoCA)and the degree of vascular cognitive impairment,they were divided into non-dementia vascular cognitive impairment(VCIND)group,vascular dementia(VD)group,and cognitively normal(CN)group.The expression level of NRG1 in serum was detected by enzyme-linked immunosorbent assay;according to the results of MRI examination,the total load score of CSVD imaging markers was calculated.The expression level of serum NRG1,MRI total load score,and CSVD cognitive impairment were analyzed for correlation.A predictive model for vascular cognitive impairment was constructed with NRG1 and MRI load.Results The average serum NRG1 expression level in the VD group was significantly higher than that in the CN group,and the difference was statistically significant(P<0.05).Pearson analysis showed that the expression level of NRG1 in serum,MRI total load score,and MoCA score in the three groups were negatively correlated,which was statistically significant(P<0.05).Logistic regression analysis showed that serum NRG1 and MRI total load were independent risk factors for the decline of cognitive function in CSVD.Receiver operating characteristic(ROC)curve analysis showed that the single and combined detection of serum NRG1 and MRI total load value had a high diagnostic value for cognitive impairment caused by CSVD,which was statistically significant(P<0.05);among them,the diagnostic value of combined detection was the highest(AUC=0.923).Conclusion The combined detection of serum NRG1 and MRI total load has a good predictive value for cognitive impairment in CSVD.
