Application Effect of Thromboelastography in Clinical Rational Blood Use in Hospitalized Severe Patients with Multiple Transfusions
Objective To explore the application effect of thromboelastography in clinical rational blood use in hospitalized severe patients with multiple transfusions.Methods A total of 200 hospitalized severe patients with a large possibility of transfusion treated in Tianshui First People's Hospital from March 2022 to March 2023 were selected.According to the random number table method,the patients were equally and randomly divided into two groups.The control group was given routine coagulation function examination,and the observation group was given routine coagulation function combined with thromboelastography examination.The blood transfusion components and blood transfusion volume indicators of the two groups were compared,and the coagulation function of the two groups of patients after transfusion was evaluated.The use of blood products by the two groups of patients and the R value,K value,α angle,and maximum amplitude(MA value)of the thromboelastogram before and after transfusion were observed.Results During the treatment,there was no statistically significant difference in the number of cases of fresh frozen plasma,cryoprecipitate,and platelet transfusion between the two groups of patients(P>0.05).After treatment,the prothrombin time(PT),activated partial thromboplastin time(APTT),and thrombin time(TT)of the observation group were higher than those of the control group,and the plasma fibrinogen(FIB)was lower than that of the control group,with statistically significant differences(P<0.05).The average usage of plasma and red blood cell suspension in the observation group was lower than that in the control group,and the average usage of platelets and cryoprecipitate was higher than that in the control group,with statistically significant differences(P<0.05).After transfusion,the R value and K value of the observation group were lower than before transfusion,the α angle was larger than before transfusion,and the MA value was higher than before transfusion,with statistically significant differences(P<0.05).Conclusion Thromboelastography plays an important role in the blood management of hospitalized severe patients with multiple transfusions.It can provide a reference for the rational use of blood for patients and test the effect of transfusion,providing a guarantee for the rational application of blood in clinical practice.
