Comparison of Clinical Effects between Single Port Laparoscopic Myomectomy via Umbilicus and Open Myomectomy in the Treatment of Uterine Fibroids
Objective To compare the clinical effects of single port laparoscopic myomectomy via umbilicus(SPLM)and open myomectomy in the treatment of patients with uterine fibroids(UF).Methods 60 patients with UF who were admitted to Puyang Oilfield General Hospital from January 2020 to December 2023 were selected.They were divided into a research group(n=30)and a control group(n=30)according to different surgical methods.The control group was treated with open myomectomy,and the research group was treated with SPLM.The perioperative indicators,hormone levels,ability to live,menstrual volume,and complications of the two groups were compared.Results The operation time of the research group was longer than that of the control group,and the first time to get out of bed,the first time to defecate,and the hospital stay after surgery were shorter than those of the control group.The visual analogue scale(VAS)score 12 hours after surgery was lower than that of the control group,and the difference was statistically significant(P<0.05).One month after surgery,the levels of follicle-stimulating hormone(FSH),estradiol(E2),and luteinizing hormone(LH)in both groups were lower than before surgery,and the research group was lower than the control group,and the differences were statistically significant(P<0.05).One month after surgery,the activities of daily living(ADL)score of both groups was higher than before surgery,and the research group was higher than the control group,and the differences were statistically significant(P<0.05).One month and three months after surgery,the menstrual volume of both groups was lower than before surgery,and the differences were statistically significant(P<0.05),but there was no statistical significance between the research group and the control group(P>0.05).The incidence of complications in the research group(3.33%)was not statistically significant compared with the control group(13.33%)(P>0.05).Conclusion Both SPLM and open myomectomy can reduce menstrual volume in patients with UF,and the safety is high.Compared with open myomectomy,SPLM has a longer operation time,but it can shorten the recovery time and hospital stay after surgery,improve hormone levels and ability to live,and relieve pain.
