Clinical Effect of Alveolar Lavage Combined with Fiberoptic Bronchoscopy Sputum Suction in the Treatment of Severe Pneumonia
Objective To explore the clinical effect of alveolar lavage combined with fiberoptic bronchoscopy(FOB)sputum suction in the treatment of severe pneumonia(SP)patients.Methods 60 SP patients treated in The First People's Hospital of Shanxian County,Sanmenxia City,Henan Province from July 2019 to April 2024 were selected.They were divided into a research group(n=30)and a control group(n=30)according to the random number table method.Both groups were given symptomatic treatment.The control group was treated with routine sputum suction,and the research group was treated with alveolar lavage combined with FOB sputum suction.The clinical efficacy,blood gas analysis indicators,inflammatory indicators,respiratory function,disease severity,and clinical indicators of the two groups were compared.Results The effective rate of treatment in the research group(93.33%)was higher than that in the control group(73.33%),and the difference was statistically significant(P<0.05).After treatment,the arterial blood oxygen saturation(SaO2),arterial blood oxygen partial pressure(PaO2),pH value,and oxygenation index(PaO2/FiO2)of the two groups were higher than before treatment,and the arterial blood carbon dioxide partial pressure(PaCO2)was lower than before treatment,and the difference was statistically significant(P<0.05).After treatment,the PaO2,SaO2,pH value,and PaO2/FiO2 levels of the research group were higher than those of the control group,and the PaCO2 level was lower than that of the control group,and the difference was statistically significant(P<0.05).After treatment,the procalcitonin(PCT),white blood cell count(WBC),and C-reactive protein(CRP)levels of the two groups were lower than before treatment,and the difference was statistically significant(P<0.05).After treatment,the PCT,WBC,and CRP levels of the research group were lower than those of the control group,and the difference was statistically significant(P<0.05).After treatment,the work of breathing(WOB),clinical pulmonary infection score(CPIS),and acute physiology and chronic health evaluation II(APACHE II)score of the two groups were lower than before treatment,and the difference was statistically significant(P<0.05).After treatment,the WOB level,CPIS,and APACHE II score of the research group were lower than those of the control group,and the difference was statistically significant(P<0.05).The disappearance of lung wet rales,cough,fever,mechanical ventilation,and ICU care time in the research group were shorter than those in the control group,and the difference was statistically significant(P<0.05).Conclusion The application of alveolar lavage combined with FOB sputum suction in the treatment of SP can improve the clinical efficacy,improve respiratory function,blood gas analysis indicators,alleviate inflammatory response,reduce symptom effects,shorten patient recovery time,and is worth clinical application.
