Objective To evaluate group sex behavior and related influencing factors among men who have sex with men(MSM)and provide a reference for formulating targeted strategies for HIV/AIDS prevention and control. Methods MSM were recruited in six cities in Shandong Province from April to July 2022.A face-to-face questionnaire was implemented to collect demographic characteristics, and behaviors of the high-risk population.Blood specimens were collected for HIV and syphilis antibodies detection. Results A total of 2 334 MSM were included in the study with the following characteristics:age,31.2±9.7 years;unmarried or divorced,1 773(76.0%);and with undergraduate degree or higher,1 454(62.3%).Of all participants,43.6%(n=l 018)self-identified as having no or low risk of HIV infection, and 16.0%(n=373)had used synthetic drugs.HIV and syphilis antibody positive rates were 3.4%(n=80)and 4.4%(n=103),respectively.A total of 19.7%(n=459)had group sex,and 51.2%(n=1 196)had consistently used condoms during homosexual intercourse in the recent 6 months. On multivariate Logistic regression analysis,MSM who had group sex behaviors in the past 6 months were more likely to be unmarried, divorced,or widowed(OR=1.45,95%CI:1.04-2.04),registered residence in another province(OR=1.43,95%CI:1.02-2.00),the main partner of anal sex was a commercial or temporary partner(OR=3.32,95%CI:2.13-5.20;OR=1.53,95%CI:1.20-1.97),had unprotected anal sex in the last 6 months(OR=1.35,95%CI:1.27-1.44),ever used synthetic drugs(OR=1.58,95%CI:1.44-1.76)and had suffered from sexually transmitted diseases in the previous year(OR=2.17,95%CI:1.54-3.03).Conclusions The proportion of MSM in Shandong Province who engage in group sexual activities is relatively high, indicating a broad presence of high-risk behaviors. It is important to strengthen surveillance and effective intervention programs for MSM who had group sex.
