目的 探究miR-497-5p、miR-3607-3p与宫颈癌合并高危型HPV感染的相关性分析.方法 选取2018年7月至2023年7月上海交通大学医学院苏州九龙医院妇科82例行宫颈活检患者临床资料进行回顾性研究,分为慢性宫颈炎组(20例)、宫颈上皮内瘤变(CIN)组(34例)、宫颈癌组(28例),另按2:1选取同期体检健康人群41例为健康对照组,比较各组的miR-497-5p、miR-3607-3p表达与高危型HPV感染差异,分析miR-497-5p、miR-3607-3p表达与高危型HPV感染的关系;比较宫颈癌临床不同病理特征患者miR-497-5p、miR-3607-3p表达;通过ROC曲线分析miR-497-5p、miR-3607-3p表达诊断高危型HPV感染合并宫颈癌的价值.结果 miR-497-5p、miR-3607-3p表达在四组中的排序依次为宫颈癌组<CIN组<慢性宫颈炎组<健康对照组(P<0.05),高危型HPV感染率在四组中的排序依次为宫颈癌组>CIN组>宫颈炎组>健康对照组(P<0.05),且随着宫颈病变加重,高危型HPV16、18型别感染发生率逐渐增加,其他型别逐渐降低(P<0.05).miR-497-5p、miR-3607-3p表达均与高危型HPV感染呈负相关(r=-0.793、-0.697,P<0.05).不同FIGO分期、脉管或宫旁转移、淋巴结转移宫颈癌患者miR-497-5p、miR-3607-3p表达比较差异有统计学意义(P<0.05),ROC曲线分析显示,miR-497-5p、miR-3607-3p表达诊断高危型HPV感染合并宫颈癌的ROC 曲线下面积(AUC)分别为 0.829、0.807;诊断敏感度、特异度:miR-497-5p 为 92.9%、83.3%;miR-3607-3p 为100.0%、64.8%,诊断具有一定价值.两项联合诊断高危型HPV感染合并宫颈癌的AUC为0.907,敏感度为100.0%、特异性为77.8%.结论 miR-497-5p、miR-3607-3p在宫颈癌中低表达,与高危型HPV感染及宫颈癌临床病理特征相关密切,可作为诊断宫颈癌合并高危型HPV感染的有效指标,且两者联合诊断价值更高.
Correlation analysis of miR-497-5p and miR-3607-3p with cervical cancer infected with high-risk HPV
Objective To explore the correlation between miR-497-5p,miR-3607-3p,and high-risk human papillomavirus(HPV)infection in cervical cancer.Methods A retrospective study was conducted on the clinical data of 82 patients who underwent cervical biopsy in the gynecology department of Suzhou Kowloon Hospital,School of Medicine,Shanghai Jiao Tong University,from July 2018 to July 2023.The patients were divided into three groups:chronic cervical inflammation(n=20),cervical intraepithelial neoplasia(CIN)(n=34),and cervical cancer(n=28).In addition,41 healthy individuals who underwent a physical examination during the same period were selected as a control group at a ratio of 2:1.The expression of miR-497-5p and miR-3607-3p in each group was compared with that in the patients with high-risk HPV infection,and the relationship between miR-497-5p and miR-3607-3p expression and high-risk HPV infection was analyzed.The expression of miR-497-5p and miR-3607-3p in patients with different clinical pathological characteristics of cervical cancer was compared.The diagnostic value of miR-497-5p and miR-3607-3p expression for cervical cancer infected with high-risk HPV was evaluated through ROC curve analysis.Results The order of miR-497-5p and miR-3607-3p expression in the four groups was cervical cancer group<CIN group<chronic cervical inflammation group<healthy control group(P<0.05),and the order of high-risk HPV infection rate in the four groups was cervical cancer group>CIN group>chronic cervical inflammation group>healthy control group(P<0.05).With the worsening of cervical disease,the incidence of HPV infection involving types 16 and 18 gradually increased,while the incidence of HPV infection with other types gradually decreased(<0.05).The expression of miR-497-5p and miR-3607-3p was negatively correlated with high-risk HPV infection(r--0.793,-0.697,P<0.05).There were statistically significant differences(P<0.05)in the expression of miR-497-5p and miR-3607-3p among cervical cancer patients with different FIGO stages,vascular or parametrial metastases,and lymph node metastases.ROC curve analysis showed that the area under the curve(AUC)scores of miR-497-5p and miR-3607-3p expression for the diagnosis of cervical cancer infected with high-risk HPV were 0.829 and 0.807,respectively.The diagnostic sensitivity and specificity for miR-497-5p were 92.9%and 83.3%,and for miR-3607-3p were 100.0%and 64.8%,respectively,indicating a certain diagnostic value.The AUC of two joint diagnoses of cervical cancer with high-risk HPV was 0.907,with a sensitivity of 100.0%and a specificity of 77.8%.Conclusions miR-497-5p and miR-3607-3p are expressed at low levels in cervical cancer,and are closely related to high-risk HPV infection and clinical pathological characteristics of cervical cancer.They can be used as effective indicators for the diagnosis of cervical cancer with high-risk HPV infection,and their combined diagnostic value is higher.
