目的 通过Meta整合方法系统梳理关于HIV感染儿童(CLH)照顾者病情告知的质性研究,为完善CLH阳性告知模式提供参考依据.方法 计算机检索中国知网、万方数据库、维普数据库、中国生物医学文献数据库、PubMed、Embase、Web of Science、Cochrane Library等数据库中有关CLH照顾者病情告知的质性研究,检索时限为建库至2023年8月.结果 最终纳入19篇文章,通过Meta整合共得出24个研究结果,其中相近结果可归类为8个新类别,最终归纳出3个整合结果:照顾者对病情告知的意愿;照顾者病情告知的困境;照顾者病情告知的经验.结论 CLH照顾者对病情告知的态度各不相同,在向HIV感染儿童进行病情告知时面临一定的困境,也总结出了一定的经验.医务人员应全面动态评估照顾者病情告知态度及需求,针对照顾者病情告知的困境,结合照顾者病情告知的经验,制定符合家庭现实的病情告知策略.
Meta-synthesis of qualitative studies on attitudes and dilemmas of caregivers'disclosure of HIV status for children living with HIV
Objective This meta-synthesis was aimed to systematically analyse qualitative research on caregivers'disclosure of HIV status for children living with HIV(CLH).In order to develop a reference point for improving HIV status disclosure methods for CLH.Methods A comprehensive search was conducted from databases including CNKI,Wanfang,CQVIP,SinoMed,PubMed,Embase,Web of Science,and Cochrane Library,and qualitative studies on caregivers'disclosure of HIV status to CLH were retrieved.The search timeframe spanned from database inception to August 2023.Results Nineteen articles met the criteria,yielding 24 research findings through meta-synthesis.These findings were grouped into eight categories,further synthesized into three key integrative themes:caregivers'willingness to disclose HIV status,difficulties encountered during HIV status disclosure,and caregivers'experiences with HIV status disclosure.Conclusions Caregivers demonstrated diverse attitudes and encountered specific difficulties when disclosing HIV status to CLH.We summarized their experiences to offer insights for healthcare professionals.To personalise disclosure strategies for families,healthcare professionals should conduct comprehensive and dynamic assessments of caregivers'attitudes and needs regarding HIV disclosure to CLH.This tailored approach,informed by caregivers'specific challenges and experiences,can optimize disclosure methodologies and support positive outcomes for CLH.
