Objective To comprehend the dropout situation and reasons for positive screening in provider-initiated testing and counseling(PITC)individuals in Chengdu and to establish a scientific foundation for improving the success rate of HIV detection.Methods HIV testing historical records from various medical institutions in Chengdu were downloaded up to the end of 2022 from the AIDS Prevention and Control Information System.This data acquisition facilitated the compilation of the number of HIV screening positive cases and confirmed testing individuals among populations undergoing PITC from 2018 to 2022 across these medical entities.A specialized table was designed to collect data on individuals with positive HIV screenings confirmed through diagnostic testing within the PITC population across all medical institutions in the 23 districts(cities and counties)of Chengdu for the year 2022.Statistical analysis was performed on the proportion and causes of loss to follow-up among these cases.Results The crude confirmation rates from 2018 to 2022 were 73.27%,50.82%,44.04%,35.72%and 33.15%,respectively,demonstrating a consistent decreasing trend over the years(x2d=7630.41,P<0.001).In 2022,the adjusted confirmed detection rate reached 97.64%(9 815/10 052)with a corresponding dropout rate of 2.36%(237/10 052).A statistically significant difference in dropout rates was observed among various medical institutions and regions(x2=46.05,P<0.001;x2=10.37,P<0.01),and private medical institutions(3.57%)and suburban areas(3.14%)exhibited the highest dropout rates,respectively.The main reason for dropout was the refusal of confirmation tests by screen-positive patients(108 cases,45.57%).Among these cases,79 were male(73.15%),53 were at least 50 years old(49.07%),and 65 were currently residing in Chengdu(60.19%).Conclusions The dropout rate among participants in PITC in Chengdu was low.However,private medical institutions had higher dropout rates than other medical facilities.The testing workflow needs further standardization to improve the success rate of testing.We need to strengthen the publicity surrounding the testing process for local individuals aged 50 years and above,particularly middle-aged and elderly people.Emphasis should be placed on the benefits of early detection,early confirmation,and early treatment to improve compliance with the confirmation detection process.
