Objective To analyze the trends of change in the volume of HIV antibody testing and patients with HIV/AIDS detected by sexually transmitted disease(STD)clinics in Sichuan Province,in order to provide a reference for further optimizing and expanding screening strategies.Methods Record cards of patients with HIV/AIDS and HIV antibody testing database were downloaded from the Chinese Disease Prevention and Control Information System.The trend analysis of composition changes for different characteristics was conducted using the Cochran-Armitage trend test,and the Joinpoint regression model was used for the trend analysis of changes such as those in the number of HIV/AIDS patients reported and testing volume.Results From 2011 to 2022,the number of HIV antibody tests performed in STD clinics in Sichuan Province was 2.065 1 million,accounting for 0.90%of the total volume from various sources in the province.The testing volume showed an upward trend,with an average annual percent change(AAPC)of 14.48%(95%CI:12.17%-16.24%),whereas its share of the total volume showed a downward trend over the years(Z=-365.81,P<0.001).The total effective detection rate(number of newly reported cases/testing volume)of STD clinics was 4.95‰,showing a downward trend and an AAPC of-9.08%(95%CI:-11.22%-6.83%).A total of 10 223 patients with HIV/AIDS were detected in STD clinics,accounting for 4.13%of the total cases detected in the province,and its proportion has been decreasing over the years(Z=-16.20,P<0.001).The number of patients detected in STD clinics showed an upward trend,with an AAPC of 5.57%(95%CI:3.03%-8.34%).Among these patients,the age group of 15-29 years represented the highest proportion(31.61%),with a predominance of male patients(81.85%).Heterosexual transmission was the primary route of transmission,accounting for 71.55%,while patients with an initial CD4 count below 200 cells/µL comprised 23.84%.Additionally,65.34%of the reporting areas and 50.32%of residences were located in Chengdu City.Trend analysis of the composition ratio revealed an increasing proportion of patients with college education or higher(Z=6.94,P<0.001).In contrast,a decrease in the proportion of patients within the 15-29 years age group(Z=-5.81,P<0.001)and those with homosexual transmission(Z=-2.36,P<0.05)was observed.The trend analysis of patient numbers indicated an upward trend for patients aged 15-29 years(AAPC=6.25%,95%CI:3.73%-8.76%),with heterosexual transmission(AAPC=3.72%,95%CI:0.98%-6.30%),and with an initial CD4 count of 500 cells/μL or more(AAPC=6.12%,95%CI:3.44%-8.68%).Conclusions From 2011 to 2022,the HIV antibody testing volume and the number of patients with HIV/AIDS detected in STD clinics in Sichuan Province showed an overall upward trend,with a relatively high proportion of young patients detected.However,the overall effective detection rate exhibited a downward trend.It is necessary for STD clinics at all levels of healthcare institutions to continuously improve the accuracy of testing targets,mobilize all patients diagnosed with STDs and with a history of high-risk sexual behavior to undergo HIV testing and maximize the detection of infected individuals.
