Hepatitis C sentinel surveillance in Jiangsu Province
Objective To analyze hepatitis C virus(HCV)sentinel surveillance in Jiangsu Province since the implementation of the Action Plan for the Elimination of Public Health Hazards of Hepatitis C(2021-2030)(referred to as the"Elimination Plan"),and to provide a reference for further implementation of HCV elimination efforts.Methods Data from HIV sentinel surveillance and HCV sentinel surveillance(including HCV population sentinel surveillance and HCV hospital sentinel surveillance)was utilized to analyze the levels of anti-HCV antibodies(anti-HCV),anti-HCV positivity rate,HCV nucleic acid detection,HCV nucleic acid positivity rate,and the proportion of antiviral treatment among different surveillance populations.Time trends were analyzed using the Cochran-Armitage trend test.Results From 2014 to 2023,among the HIV sentinel surveillance sites in Jiangsu Province,the highest anti-HCV positivity rate was observed among drug users(18.42%),and the anti-HCV positivity rate among drug users decreased from 28.50%in 2014 to 14.62%in 2023,with a decreasing trend(Z=16.06,P<0.001).From 2021 to 2023,the highest anti-HCV positivity rates were observed in the hemodialysis subpopulation(3.26%)and in the infectious diseases department(2.91%).The proportion of newly reported HCV cases undergoing HCV nucleic acid testing in the HCV hospital sentinel surveillance increased from 89.47%in 2021 to 96.55%in 2023(Z=-2.39,P=0.017),and the proportion of newly reported HCV cases initiating antiviral treatment increased from 29.82%in 2021 to 62.64%in 2023(Z=-6.06,P<0.001).Conclusions Drug users and hemodialysis population are the major subpopulation for HCV prevention and control in Jiangsu Province.The strategies of"all tests for HCV if needed"and"full coverage of HCV nucleic acid testing if HCV antibody positive"can improve the screening and detection of current HCV-infected individuals in key departments of medical institutions.Efforts should be made to strengthen antiviral treatment for current HCV infections to achieve the goal of HCV elimination.
