The prevalence and influencing factors of high-risk sexual behaviors among adolescents with self-reported sexual behavior in Chongqing
Objective This study aimed to investigate the prevalence of and factors influencing high-risk sexual behaviors, such as multiple sexual partners, unprotected sex,and coerced sexual behavior, among self-reported sexually active adolescents. Methods A cross-sectional survey was conducted using a questionnaire among sexually active adolescents at a university and high schools in Chongqing. The chi-square test was used for group comparisons of count data, and multivariate logistic regression analysis was performed to identify factors associated with having multiple sexual partners, engaging in unprotected sex,and experiencing coerced sexual behavior. Results A total of 6 786 adolescents were surveyed, and the prevalence of sexual activity was 10.6%(722/6 786).Among the sexually active adolescents,31.2%(225/722)reported having multiple sexual partners,31.7%(229/722)engaged in unprotected sex,and 8.4%(61/722)experienced coerced sexual behaviors. The chi-square test showed that the proportion of adolescents with multiple sexual partners was higher in males(33.5%,172/513)than in females(25.4%,53/209).The proportion of adolescents who engaged in unprotected sex was higher in males(34.7%,178/513)than in females(24.4%,51/209),and higher in high school students(50.2%,139/277)than in university students(20.2%,90/445);rural students(37.1%,179/482)was higher than urban students(20.8%,50/240).The proportion of adolescents who experienced coerced sexual behavior was higher among high school students(13.4%,37/277)than among university students(5.4%,24/445).These differences were considered statistically significant(P<0.05).Multivariate logistic regression analysis revealed that adolescents with a higher level of sensation-seeking were more likely to have multiple sexual partners(OR=1.190,95%CI:1.126-1.259),and those with poor parental discipline were more likely to engage in unprotected sex(OR=0.970,95%CI:0.946-0.995).Adolescents who had experienced childhood abuse(OR=1.105,95%CI:1.002-1.219)and those with a higher level of sensation-seeking behavior(OR=1.154,95%CI:1.050-1.268)were more likely to experience coerced sexual behavior. Conclusions A certain proportion of self-reported sexually active adolescents in Chongqing engaged in high-risk sexual behaviors. When developing and implementing education programs on high-risk sexual behaviors, attention should be paid to males, high school students, rural students, individuals with high levels of sensation-seeking behavior, those with poor parental discipline, and those who have experienced childhood abuse.
