摘要
目的 分析一起新型冠状病毒(新冠病毒)感染聚集性疫情的流行病学特征,为传染病防控提供科学依据.方法 收集2022年7月28日至8月5日广西崇左市宁明县一起新型冠状病毒感染疫情现场流行病学调查数据,采集病例及密切接触的鼻咽/口咽拭子并进行全基因组序列比对分析.采用Excel 2010对数据进行处理和分析,使用R软件计算基本再生系数(R0)和实时再生系数(Rt).结果 该起聚集性疫情的指示病例为某货车司机,其在2022年7月26-29 日期间累计感染14人,并导致了疫情的扩散蔓延.整起疫情最少传播1代,最多传播4代,累计感染38人;时间分布上存在2个高峰,分别为7月31日和8月3日;年龄以30~60岁为主(26/38,68.42%),职业以餐饮服务人员为主(9/38,23.68%);发现方式以管控人员为主(30/38,78.95%),非管控人员主要在疫情的早期被发现;病例接触感染源后可检出新冠病毒核酸阳性的时间在2~5 d;该起疫情流行株为奥密克戎变异株BA.5.2的进化分支毒株,R0为6.79(95%CI:3.33~12.07),Rt先上升后下降.结论 本起疫情是由一货车司机引起的奥密克戎病毒超级传播,应做好货车司机,尤其是边境地区货车司机的管理,以减少类似疫情的扩散蔓延.
Abstract
Objective To analyze the epidemiological characteristics of a COVID-19 cluster epidemic in Ningming county,Guangxi,and to provide scientific basis for the prevention and control of similar infectious diseases in the future.Methods Data were collected from the field epidemiological survey conducted from July 28 to August 5,2022,in Ningming county,Guangxi.Oral and pharyngeal swabs of positive cases were collected for whole-genome sequence comparison.Data were processed and analyzed using Excel 2010,and the basic regeneration coefficient(R0)and real-time regeneration coefficient(Rt)were calculated using R software.Results The initial case of the cluster was a truck driver,who transmitted to 14 individuals during July 26-29,2022,leading to the spread of the outbreak.The epidemic spread across at least 1 generation and up to 4 generations,affecting a total of 38 positive cases.In terms of time distribution,there were two peaks,which were July 31 and August 3.The age group was mainly 30-60 years old(26/38,68.42%),and the occupation group was mainly catering service staff(9/38,23.68%).The SARS-CoV-2-positive person was mainly control personnel(30/38,78.95%),and non-control personnel were mainly concentrated in the early stage of the epidemic.Detection of positive cases occurred between 2 and 5 days.The epidemic strain identified was the Omicron variant BA.5.2,with an R0 of 6.79(95%CI,3.33-12.07),and Rt initially increased and then decreased.Conclusion The super spread of Omicron is initiated by a truck driver.Effective management of truck drivers,especially those in border areas,should be done well to reduce the spread of similar epidemics.
