摘要
目的 分析世界卫生组织(World Heath Organzation,WHO)194个成员国轮状病毒(rotavirus,RV)疫苗纳入国家免疫规划(National Immunization Program,NIP)的现况,为优化调整我国轮状病毒疫苗免疫策略提供科学依据.方法 收集截至2022年WHO 6大区域内194个成员国的轮状病毒疫苗免疫程序、疫苗类型等公开数据,并对轮状病毒免疫策略进行描述性分析.结果 194个WHO成员国中,有124国NIP至少纳入1种轮状病毒疫苗,83国纳入NIP的是单价轮状病毒疫苗,分别实施1剂(2国)、2剂(76国)和3剂(5国)免疫程序;32国纳入NIP的是五价轮状病毒疫苗,分别实施1剂(1国)、2剂(2国)、3剂(28国)和4剂(1国)免疫程序,9国纳入NIP的是单价轮状病毒疫苗/五价轮状病毒疫苗,均实施3剂免疫程序.轮状病毒疫苗接种率不同区域差别很大,2022年轮状病毒疫苗接种率为4％～74％.结论 大部分国家已将轮状病毒疫苗纳入NIP、不同区域疫苗接种率差异很大,我国应充分考虑国内轮状病毒相关疾病的负担,加强疾病监测,疫苗应用效果分析,加快评估轮状病毒疫苗纳入我国NIP的进程.
Abstract
Objective To analyze the current status of rotavirus(RV)vaccines in the National Immunization Program(NIP)of 194 member countries of the World Health Organization(WHO),and to provide a scientific basis for optimizing and adjusting RV vaccine immunization strategies in China.Methods Public data on RV vaccine immunization procedures and vaccine types were collected from 194 member countries across six regions of WHO,and descriptive analysis was performed.Results Among the 194 WHO member countries,124 had at least one type of RV vaccine in their NIP.Of these,83 countries had adopted monovalent RV vaccines,implementing immunization schedules of 1 dose(2 countries),2 doses(76 countries),and 3 doses(5 countries).Additionally,32 countries had incorporated pentavalent RV vaccines,with immunization schedules of 1 dose(1 country),2 doses(2 countries),3 doses(28 countries),and 4 doses(1 country).Nine countries utilized both monovalent and pentavalent vaccines,all implementing a 3-dose schedule.The RV vaccine coverage rates for RV vaccines varied significantly by region,with coverage ranging from 4％to 74％in 2022.Conclusion The majority of countries have incorporated RV into NIP;however,there are substantial differences in vaccination coverage across regions.China should fully consider the burden of domestic RV-related diseases,strengthen disease surveillance,analyze the effect of vaccine application,and expedite the evaluation process for including RV vaccines in NIP.
