彩色多普勒超声在肠系膜上动脉栓塞诊断中的应用

摘要
目的：探讨彩色多普勒超声在诊断肠系膜上动脉栓塞中的应用价值。材料和方法：回顾性分析经超声检查并经造影证实的8例肠系膜上动脉栓塞的声像图表现和血流参数，并与健康志愿者对比。结果8例确诊患者中彩超发现血栓7例，5例血栓位于肠系膜上动脉起始部，2例位于肠系膜上动脉第一分支处。彩色多普勒超声显示4例栓塞局部无血流充盈，3例可见充盈缺损。1例患者肠系膜上动脉近段管腔未能发现血栓，但起始部血流阻力明显增高。实验组近端肠系膜上动脉血流速度较正常组低（P＜0．05），阻力指数（RI）高于正常组（P＜0．05）。结论肠系膜上动脉栓塞具有特异的声像图表现，彩色多普勒超声可作为诊断的首选检查方法。
Abstract
Objective To analyze the value of Color Doppler ultrasonography in diagnosis the thrombus of superior mesenteric artery embolism .Methods To Retro-spective analysis the sonographic images of eight cases with superior mesenteric artery embolism ,which had been confirmed by angiography.The hemodynamic parameters of these patients were compared with healthy volunteers.Results 7 cases in 8 patients were diagnosed by Doppler ultrasound accurately.Five cases of thrombosis in the beginning part of the superior mesenteric artery,two cases in the first branch of superior mesenteric artery.Color Doppler ultrasound showed four cases of embolism were no local flow filling,three cases were filling defect.Superior mesenteric artery thrombus was not being found in one patient,but the beginning part of the resistance to blood flow was significantly higher.Proximal superior mesenteric artery blood flow velocity was lower than the normal group(P<0.05),RI was higher than the normal group(P<0.05).Conclusion Color Doppler ultrasound could be the first choice in the diagnosis of Superior mesenteric artery embolism because specific sonographic will be showed.
