摘要
目的:调查上海市浦东新区惠南社区卫生服务中心家庭病床服务现状,以期为辖区老年患者带来更优质的家庭病床服务.方法:描述性分析2023年惠南社区卫生服务中心753例建立了家庭病床患者的年龄、性别、支付方式、疾病构成、疾病转归、家庭医生签约情况等信息.结果:753例家庭病床患者平均年龄为(74.0±8.8)岁,60岁及以上老年人占98.41%;家庭病床服务支付方式以城镇职工基本医疗保险为主,占86.99%,其次是城镇居民基本医疗保险(10.89%).家庭病床服务患者以慢性病患者为主,病种排名前3位的疾病分别为高血压(75.96%)、糖尿病(20.72%)、冠心病(18.99%).家庭病床患者罹患1种、2种及3种疾病的患者分别占60.60%、22.60%及11.00%.家庭病床患者病情转归情况以好转和稳定为主,两者合计占比达94.90%.结论:浦东新区家庭病床服务有效地促进了医疗资源合理利用,医保政策的支持对于患者选择家庭病床服务具有重要推动作用.但家庭病床服务宣传缺乏其他部门协同,后期需要通过政策引导、医保支持、加大宣传等多方面的努力,进一步推动家庭病床服务的发展.
Abstract
OBJECTIVE To investigate the service status of the family sickbed in Huinan Community Health Service Center,in order to bring better quality home bed service for elderly patients.METHODS It analyzed the age,gender,payment method,disease composition,disease outcome,family doctor contract and other information of the patients who established family sickbeds in the center from January to December 2023,a total of 753 people.RESULTS 98.41%of 753 patients were 60 years old and above,with an average age of 74.0±8.8 years.In terms of payment methods,86.99%were basic medical insurance for urban workers,followed by basic medical insurance for urban residents(10.89%)and other payment methods.In terms of disease structure,the patients with home bed were mainly chronic diseases,among which hypertension(75.96%),diabetes(20.72%)and coronary heart disease(18.99%)were the top three diseases.At the same time,patients with home bed also had multiple diseases,and patients with 1,2 and 3 diseases were 60.60%,22.60%and 11.00%,respectively.Among the discharge outcomes of patients at home,94.90%of patients were discharged because their condition was stable or improved.CONCLUSION Family sicked service effectively promotes the rational use of medical resources,and the support of medical insurance policies plays an important role in promoting patients'choice of family sickbed service.However,the publicity of Family sickbed lacks the coordination of other departments,and it is hoped that the following efforts can be made through policy guidance,medical insurance support,and more publicity.Further promote the development of family sickbed services.
