Impact of Medical-nursing Combination Services via the"Health Cloud"Platform on the Quality of Life and Health-care Resource Utilisation of Elderly Community Patients with Chronic Diseases
OBJECTIVE To analyse the impact of integrated medical and nursing services conducted via the"Health Cloud"platform on the quality of life and utilisation of health resources by elderly patients with chronic diseases in the community.METH-ODS A total of 103 elderly community patients with chronic diseases were selected,using a self-controlled trial design.The control period was from September 2022 to March 2023,following routine medical procedures,while from April 2023 to July 2024,the in-tegrated services were supported by the"Health Cloud"platform.The quality of life and health resource utilisation before and after the intervention were evaluated T0 was the beiganing of control period,T1 is the end of control period,T3 was intervention period,T4 was the end of intervention period,RESULTS 98 patients completing the entire study for analysis.Comparisons of patients'quality of life at different periods revealed statistically significant differences(P<0.05),exhibiting an overall upward trend.During the con-trol period at T4,quality of life scores in all dimensions increased compared to T3,T2,T1,T0,with statistically significant differ-ences(P<0.05).In the intervention period,the number of family doctor service appointments,usage of community rehabilitation services,utilisation of community health management services,chronic disease follow-ups,and participation in health education from T3-T4 were higher than T2-T3 and the control period,with statistically significant differences(P<0.01).During the intervention period,the referral rate from T3-T4 decreased compared to T2-T3 and the control period;the completion rates of physical examina-tions and vaccination rates in T2-T3 increased compared to the control period,with statistically significant differences(P<0.05).CONCLUSION The"Health Cloud"platform's integrated medical and nursing service model can significantly improve the quality of life of elderly patients with chronic diseases and optimise the use of healthcare resources.
