目的:分析南京市江宁区自2019年起实施8月龄麻疹腮腺炎风疹联合减毒活疫苗(以下简称麻腮风疫苗)替代麻疹风疹联合减毒活疫苗(以下简称麻风疫苗)的安全性.方法:从江苏省预防接种综合服务管理信息系统和中国免疫规划信息管理系统中导出2016-2023年江宁区8～17月龄儿童麻腮风疫苗和麻风疫苗接种数据,分析两种疫苗疑似预防接种异常反应(Adverse Events Following Immunization,AEFI)发生率.结果:2016-2023年江宁区8～17月龄儿童麻腮风疫苗AEFI报告发生率为492.32/10万,高于麻风疫苗AEFI报告发生率(106.20/10万),差异有统计学意义(χ2=220.571,P＜0.01).两种疫苗一般反应的临床表现均以一过性发热为主;麻风疫苗异常反应中报告最多的是过敏反应—荨麻疹,无血小板减少性紫癜的报告;接种麻腮风疫苗的异常反应中报告最多的是过敏性皮疹.麻腮风疫苗AEFI个案中过敏性皮疹占比高于麻风疫苗,差异有统计学意义(χ2=9.2571,P＜0.05).结论:麻风疫苗、麻腮风疫苗AEFI临床表现均以一般反应为主,其次为非严重的异常反应,麻腮风疫苗替代麻风疫苗,其安全性可靠.
Safety Analysis of Replacing Measles Rubella Combined Attenuated Live Vaccine with Measles,Mumps and Rubella Combined Attenuated Live Vaccine among Children 8 Months of Age in Jiangning from 2016 to 2023
OBJECTIVE To analyze the safety of replacing measles rubella(MR)combined attenuated live vaccine with measles,mumps and rubella(MMR)combined attenuated live vaccine among 8-month-old children in Jiangning District since 2019.METHODS It obtained MR and MMR doses administered data and adverse reaction data in age-eligible children during 2016-2023 from the Jiangsu Vaccination Integrated Service Management Information System and the China Immunization Program Information System,and conducted a descriptive analysis of reported adverse events following immunization(AEFI)rates among children 8～17 months of age.RESULTS The incidence of reported adverse events following MR administration was 106.20/105 in Jiangning District from 2016 to 2023.And the incidence of reported adverse events following MMR administration was 492.32/105.The reported incidence of MMR was higher than that of MR(χ2=220.571,P＜0.01).Among the clinical symptoms of general reactions,transient fever was the most common one.The most frequently reported abnormal reaction to MR vaccination was urticaria,no reports of thrombocytopenic purpura.The most frequently reported abnormal reaction to MMR was allergic skin rash,higher than that of MR(χ2=9.2571,P＜0.05).CONCLUSION Regardless of whether it was MR or MMR vaccination,the majority of AEFI are general reactions,followed by non-serious abnormal reactions.The incidence of MMR adverse reactions among children is within the expected range.
