摘要

目的 应用多普勒及剪切波弹性成像(SWE)评价重度主动脉瓣狭窄(AS)患者经导管主动脉瓣置换术(TAVI)后肾脏血流动力学等相关情况.方法 初次行TAVI的重度AS患者97例,术前及术后1个月测量肾脏大小、皮质厚度,各级动脉收缩期最高流速(PSV)、舒张期最低流速(EDV)、阻力指数(RI)、搏动指数(PI)及肾实质弹性模量值(E),收集临床生化指标血肌酐、胱抑素及尿素,比较各参数及生化指标术前术后差异.结果 TAVI术后肾脏各级动脉PSV、EDV升高,RI及PI下降,肾实质E降低,差异具有统计学意义(P<0.05).结论 TAVI术后肾脏血流灌注增加,多普勒及SWE对TAVI术后肾脏血流灌注评价有诊断价值.

Abstract

Objective To evaluate the renal hemodynamics and related conditions in patients with severe aortic stenosis(AS)after transcatheter aortic valve implantation(TAVI)by Doppler ultrasound and shear wave elastogra-phy(SWE).Methods A total of 97 patients with severe AS underwent TAVI for the first time were collected,and the kidney size,cortical thickness,arterial peak systolic velocity(PSV),end diastolic velocity(EDV),resistance in-dex(RI),pulsatility index(PI),and renal parenchymal elastic modulus(E)before and 1 month after surgery were measured.The clinical biochemical parameters of serum creatinine,cystatin C,and urea before and after surgery were also collected and compared.Results After TAVI,renal arterial PSV and EDV at all branches were increased,RI and PI were decreased,and renal parenchymal elastic modulus value was decreased,all with a statistically significant difference(P<0.05).Conclusions Renal blood perfusion increases after TAVI,Doppler ultrasound and SWE have diagnostic value in evaluating renal blood perfusion after TAVI.
