常规超声及超声弹性成像对淋巴瘤分期诊断作用的研究

摘要
目的 本研究旨在回顾性分析淋巴瘤不同分期的常规超声与超声弹性成像特征,探讨常规超声及超声弹性成像对于淋巴瘤分期诊断的应用价值.方法 本研究选取病理确诊为淋巴瘤的患者206例,按淋巴瘤临床分期标准(Ann Arbor分期)Ⅰ、Ⅱ期分为早期组,Ⅲ、Ⅳ期分为晚期组,对两组患者的常规超声及超声弹性成像特征进行定性和定量分析.结果 常规超声检查显示早期组与晚期组淋巴结相比,长短径比值更小,差异有统计学意义(P＜0.05),两组的长径和短径、淋巴门情况则不存在差异(P＞0.05);多普勒彩色血流显示晚期组存在边缘型或混合型的血流类型更多,差异有统计学意义(P＜0.05);超声弹性成像显示晚期组弹性为硬的更多,差异有统计学意义(P＜0.05).结论 本研究显示淋巴结的长短径比、血流类型及超声弹性成像指标在早期和晚期淋巴瘤淋巴结中有明显不同,一定程度上可以反映淋巴瘤的分期情况,有重要的临床意义及临床实用价值.
Abstract
Objective To explore the application value of conventional ultrasound and ultrasonic elastography in lymphoma staging diagnosis.Methods A total of 206 patients diagnosed with lymphoma were included in this study,and divided into two groups according to Ann Arbor staging criteria(stages Ⅰ and Ⅱ for early stage group and stages Ⅲ and Ⅳ for late stage group).The conventional ultrasound and ultrasonic elastography of these patients were retro-spectively analyzed.Results For conventional ultrasound,the ratio of long to short diameter of lymph nodes in the early stage group was smaller and statistically significant than that in the late stage group(P＜0.05).There was no significant difference in the length,short diameter,and lymph node status between the two groups(P＞0.05).Color Doppler flow showed that lymph node in the late stage group had more marginal or mixed blood flow types,and the difference was statistically significant(P＜0.05);Ultrasonic elastography showed that lymph node in the late stage group had more rigid elasticity,and the difference was statistically significant(P＜0.05).Conclusions This study shows the ratio of long to short diameter,blood flow type,and lymph node elasticity of lymph nodes are significantly different in early and late stage lymphoma lymph nodes.Conventional ultrasound and ultrasonic elastography could be important for the staging of lymphoma in clinical practical.
