摘要

目的 探讨先天性心脏病(CHD)胎儿母体螺旋动脉(SA)是否存在重塑异常,并进一步分析与胎儿血流动力学之间的潜在相关性.方法 前瞻性选取诊断为CHD的80例胎儿作为病例组,同期收集96例健康胎儿作为对照组.通过血流成像技术(Slow-flowHD)定量分析两组SA血管指数(VI),同时分析SA最大流速(PSV)和阻力指数(RI);胎儿血流动力学参数选取脐动脉(UA)、大脑中动脉(MCA)的搏动指数(PI)、脑-胎盘比(CPR)以及脐静脉血流量(UVFV).结果 病例组SA-VI和SA-PSV低于对照组,SA-RI高于对照组(P<0.01),差异均有统计学意义.SA-VI与UA-PI呈负相关,与UVFV、MCA-PI、CPR呈正相关;相反,SA-RI与UA-PI呈正相关,与UVFV、MCA-PI、CPR呈负相关.结论 先天性心脏病胎儿母体SA存在重塑异常,且与胎儿血流动力学密切相关,强调了整体评估母体、胎盘和胎儿在围产期管理中的重要性.

Abstract

Objective To investigate whether remodeling abnormalities exist in the spiral arteries(SA)of fetuses with congenital heart disease(CHD)and further analyze the potential correlation between these abnormalities and fetal hemodynamics.Methods In this prospective study,80 fetuses diagnosed with CHD through ultrasound imaging were selected as the case group,and 96 healthy fetuses were recruited as the control group during the same period.The vas-cular index(VI)of SA was quantitatively analyzed using SlowflowHD technology in both groups.Additionally,the peak systolic velocity(PSV)and resistance index(RI)of SA were evaluated.Fetal hemodynamic parameters,inclu-ding the pulsatility index(PI)of umbilical vein(UV)and middle cerebral artery(MCA),cerebroplacental ratio(CPR)and umbilicalvenous flow volume(UVFV),were also measured.Results The SA-VI and SA-PSV in the case group were significantly lower than those in the control group,while the SA-RI was significantly higher(P<0.01),indicating statistically significant differences.A negative correlation was observed between SA-VI and UA-PI,while SA-VI positively correlated with UVFV,MCA-PI,and CPR.Conversely,SA-RI positively correlated with UA-PI and negatively correlated with UVFV,MCA-PI,and CPR.Conclusions Remodeling abnormalities exist in the spiral arteries of fetuses with CHD,which are closely related to fetal hemodynamic parameters.This underscores the impor-tance of comprehensive assessment of the mother,placenta,and fetus in perinatal management.
