经阴道超声宫颈弹性成像联合胎盘微血管指数对双胎自发性早产的预测价值

摘要
目的 探究经阴道超声宫颈弹性成像(E-Cervix)联合胎盘微血管指数(MVI)对双胎自发性早产(SPTB)的预测价值.方法 选取本院定期产检且完成分娩的双胎妊娠孕妇130例,依据分娩结局将其分为SPTB组与足月产组.对比两组孕妇E-Cervix及胎盘MVI相关参数;采用受试者工作特征(ROC)曲线分析E-Cervix、胎盘MVI相关参数对双胎SPTB的预测效能.结果 SPTB组孕妇宫颈长度(CL)、硬度比值(HR)、胎盘血管化指数(VI)、血流指数(FI)、血管化-血流指数(VFI)均显著小于足月产组(P＜0.05),宫颈内口应变值(IOS)显著大于足月产组(P＜0.05).E-Cervix相关参数中,CL、IOS、HR预测SPTB的曲线下面积(AUC)分别为 0.816、0.738、0.797;胎盘 MVI 相关参数中,VI、FI、VFI 预测 SPTB 的 AUC 分别为 0.792、0.688、0.889;各项指标联合预测SPTB的AUC为0.994,灵敏度为100％,特异度为94.44％.结论 E-Cervix及胎盘MVI相关参数对双胎SPTB有一定的预测价值,相关参数联合检测能提升SPTB预测效能.
Abstract
Objective To investigate the predictive value of transvaginal ultrasound elastography-cervix(E-Cervix)combined with placental microvascular index(MVI)in twins with spontaneous preterm birth(SPTB).Methods A total of 130 pregnant women with twin pregnancies who completed the delivery after regular birth exami-nation were selected and divided into SPTB group and full-term group according to delivery outcome.E-Cervix and placental MVI related parameters were compared between the two groups;The predictive efficacy of E-Cervix and pla-cental MVI parameters for SPTB in twins was analyzed by ROC.Results The cervical length(CL),hardness ratio(HR),placental vascularization index(VI),blood flow index(FI)and vascularization flow index(VFI)in SPTB group were significantly lower than those in full-term group(P＜0.05),and the internal cervical os strain value(IOS)was significantly higher(P＜0.05).Among E-Cervix related parameters,the area under the curve(AUC)predicted by CL,IOS and HR for SPTB was 0.816,0.738 and 0.797,respectively;Among the parameters related to placenta MVI,the AUC of SPTB predicted by VI,FI and VFI were 0.792,0.688 and 0.889,respectively;The AUC of SPTB was 0.994,the sensitivity was 100.00％and the specificity was 94.44％.Conclusions E-Cervix and placen-tal MVI related parameters have certain predictive value for SPTB in twins,and the combined detection of related pa-rameters can improve the predictive efficiency of SPTB.
