摘要

目的 分析改良卵巢-附件影像报告和数据系统(O-RADS)联合糖类抗原125对卵巢肿瘤良恶性诊断的应用价值.方法 回顾性分析306个卵巢肿瘤超声声像图并进行传统O-RADS分类,对O-RADS 4类肿瘤进行二次改良O-RADS分类和联合CA125的改良O-RADS分类,以病理结果为标准绘制受试者工作特征(ROC)曲线,比较传统O-RADS、改良O-RADS、改良O-RADS联合CA125的曲线下面积(AUC),并分析3种诊断模型对卵巢肿瘤的诊断效能.结果 传统O-RADS2、3、4、5分类中卵巢恶性肿瘤发生率分别为1.0%、4.3%、41.1%、93.4%,分类越高恶性率越高,差异有统计学意义(P<0.001);改良O-RADS联合CA125诊断的AUC显著高于改良O-RADS、传统O-RADS(P<0.001),3种诊断模型的AUC分别为0.932、0.870、0.850,改良O-RADS与传统 O-RADS的AUC差异无统计学意义(Z=-1.205,P=0.228).改良O-RADS联合CA125诊断的特异度、阳性预测值、准确度高于改良O-RADS、传统O-RADS(95.9%>84.7%>74.8%,89.4%>68.8%>58.8%,94.4%>85.9%>80.3%).结论 改良O-RADS分类在卵巢肿瘤的良恶性诊断中有较高的应用价值,改良O-RADS联合CA125可进一步提高诊断效能.

Abstract

Objective To analyze the value of the modified ovarian-adnexal reporting and data system(O-RADS)combined with carbohydrate antigen 125 for the benign and malignant diagnosis of ovarian tumors.Methods Sono-grams of 306 ovarian tumors were retrospectively analyzed and classified by traditional O-RADS,the secondary modi-fied O-RADS category and modified O-RADS category combined with CA125 were performed on tumors of O-RADS categories 4.The receiver operating characteristic(ROC)curves of subjects were plotted using pathological finding as the criterion to compare the area under the curve(AUC)of traditional O-RADS,modified O-RADS,and modified O-RADS combined CA125,and to analyze the diagnostic efficacy of the three diagnostic models in ovarian tumor.Results The incidence of malignant in O-RADS categories 2,3,4,and 5 ovarian tumors classified by traditional method were 1.0%,4.3%,41.1%,and 93.4%respectively.The higher classification grade showed higher malig-nant rate,and the difference was statistically significant(P<0.001).The AUC of modified O-RADS combined with CA125 was significantly higher than that of modified O-RADS and traditional O-RADS(P<0.001),which were 0.932,0.870 and 0.850 respectively,and difference of AUC between modified O-RADS and traditional O-RADS was not statistically significant(Z=-1.205,P=0.228).The specificity,positive predictive value,and accuracy of modi-fied O-RADS combined with CA125 were higher than those of modified O-RADS and traditional O-RADS(95.9%>84.7%>74.8%,89.4%>68.8%>58.8%,94.4%>85.9%>80.3%).Conclusions The modified O-RADS catego-ry has high application value in the diagnosis of benign and malignant ovarian tumors,and the modified O-RADS com-bined with CA125 can further improve the diagnostic efficiency.
