子宫内膜息肉样病变三维超声图像特征与病理类型及性激素水平相关性分析

摘要
目的 探究三维超声与子宫内膜息肉(EP)样病变患者病理类型及性激素水平的相关性.方法 回顾性选取收治的经病理确诊的EP样病变患者120例,区分患者病理类型,比较二维与三维超声诊断EP样病变的价值;根据病理诊断结果将患者分为良性组和恶性组,比较两组一般资料、三维超声检查及性激素检查结果,采用Pearson相关系数分析三维超声参数与性激素的关系,应用多因素Logistic回归模型分析EP样病变患者发生癌前病变和癌变的影响因素.结果 120例EP样病变患者中经病理检查确诊为癌前病变(不典型增生)10例、癌变(EP样癌)30例,总发生率为33.33%;三维超声诊断EP样病变的准确度高于二维超声(P<0.05);恶性组患者绝经后阴道出血的比例、子宫内膜厚度>10 mm的比例高于良性组,子宫动脉RI低于良性组,血清雌二醇(E2)高于良性组(P<0.05);血清E2水平与子宫动脉RI呈负相关,与子宫内膜厚度和病理类型呈正相关(P<0.05);绝经后阴道出血、低子宫动脉RI、子宫内膜厚度>10 mm以及高血清E2水平是导致EP样病变患者发生恶性病变的独立危险因素(P<0.05).结论 三维超声与EP样病变患者病理类型和性激素水平密切相关,子宫动脉RI越低、子宫内膜越厚,患者血清E2也越高,且绝经后阴道出血、子宫动脉RI降低、子宫内膜厚度>10 mm以及E2水平升高是EP样病变患者发生癌前病变和癌变的独立危险因素.
Abstract
Objective To explore the correlation among 3D ultrasound,pathological types and sex hormone lev-els in patients with endometrial polyps(EP)-like lesions.Methods A total of 120 patients with pathologically con-firmed EP-like lesions were retrospectively selected.Pathological types of the patients were distinguished and the value of 2D and 3D ultrasonography in diagnosing EP-like lesions was compared.Patients were divided into the benign group and the malignant group according to the pathological diagnosis results.General data,3D ultrasound and sex hormone examination results were compared between the two groups,and Pearson correlation coefficient was used to analyze the relationship between 3D ultrasound parameters and sex hormones.Multivariate Logistic regression model was used to analyze the influencing factors of precancerous and cancerous lesions in EP patients.Results Among the 120 patients with EP-like lesions,10 cases were confirmed as precancerous lesions(atypical hyperplasia)and 30 cases were confirmed as cancerous lesions(EP-like carcinoma)by pathological examination,with a total prevalence of 33.33%.The accuracy of 3D ultrasound in the diagnosis of EP-like lesions was higher than that of 2D ultrasound(P<0.05).The proportion of postmenopausal vaginal bleeding and endometrial thickness>10 mm in the malignant group was higher than that in the benign group,uterine artery RI was lower,while serum estradiol(E2)level was higher(P<0.05).Serum E2 level was negatively correlated with uterine artery RI,and positively correlated with endometrial thickness and pathological type(P<0.05).Postmenopausal vaginal bleeding,low uterine artery RI,endometrial thickness>10 mm,serum E2 were independent risk factors for malignant lesions in patients with EP-like lesions(P<0.05).Conclusions 3D ultrasound is closely related to pathological types and sex hormone levels in patients with EP-like lesions.The lower the uterine artery RI and the thicker the endometrium,the higher the serum E2.Moreo-ver,postmenopausal vaginal bleeding,lower uterine artery RI,endometrial thickness>10 mm,and elevated E2 level are independent risk factors for precancerous lesions and carcinogenesis in patients with EP-like lesions.
