摘要

目的 探讨超声在残角子宫妊娠诊断及孕期监测中的应用价值.方法 回顾性分析11例可疑残角子宫妊娠患者临床表现、超声诊断、孕期监测及妊娠结局情况,探讨不同孕期各型残角子宫妊娠的超声特征.结果 11例可疑残角子宫妊娠患者中交通型(Ⅰ型)2例,非交通型(Ⅱ型)7例,误诊病例2例;9例确诊病例中5例于早孕期行腹腔镜下残角子宫切除术,2例中孕期行经腹部残角子宫切除术,2例动态观察至孕晚期行剖宫产及残角子宫切除术、娩出活胎;残角子宫妊娠超声征象为妊娠侧宫体与宫颈、宫颈管间不存在延续性;妊娠侧宫体与对侧宫体间可存在或不存在连接;妊娠侧宫体肌层厚度、张力情况及胎盘与肌层关系为妊娠结局提供参考.结论 残角子宫妊娠的超声征象对其诊断及孕期监测具有较好的价值.

Abstract

Objective To explore the application value of ultrasound in the diagnosis and monitoring of rudimen-tary horn pregnancy.Methods Clinical and ultrasonic data and pregnancy outcomes were analyzed retrospectively in 11 patients with suspicious rudimentary horn pregnancy,and the ultrasonic manifestations of different types of rudi-mentary horn pregnancy were summarized.Results Among 11 patients with suspicious rudimentary horn pregnancy,2 were type Ⅰ,7 were type Ⅱ and 2 misdiagnosed cases;5 underwent laparoscopic surgery in the first trimester,2 cases underwent transabdominal hysterectomy of residual horn during the second trimester,and two live fetuses were deliv-ered by cesarean section and hysterectomy of residual horn in the third trimester;The ultrasonic manifestations of ru-dimentary horn pregnancy were the absence of continuity between the uterine of pregnancy side and the cervix or cervi-cal canal,and the rudimentary horn might or might not be connected with the normal cavity;The thickness,tension of the uterine body of pregnancy,and the placenta,posterior placental space provided reference for pregnancy out-come.Conclusions Ultrasonic manifestations of rudimentary horn pregnancy have a good value for diagnosis and mo-nitoring.
