摘要
目的 探讨医共体合作下延伸护理对慢性心力衰竭(简称:慢性心衰)患者的影响,为提高患者生活质量提供帮助.方法 选取2020年1月—2022年2月在常熟市第一人民医院及其医共体医院住院且好转出院的慢性心衰患者201例,采用随机数字表法分为对照组100例和研究组101例.对照组由医院护士进行电话随访,研究组由医共体医院下属的社区医护人员进行随访,分别在患者出院后3、6及12个月进行随访.比较两组心功能分级、再住院与死亡人数、生活质量评分.结果 两组出院后3、6及12个月随访时心功能分级、再住院与死亡人数差异无统计学意义(P＞0.05).研究组出院3个月随访时躯体疼痛、一般健康状况、健康变化评分高于对照组,差异均有统计学意义(P＜0.05);研究组出院6个月随访时躯体疼痛、情感职能、健康变化评分高于对照组,差异均有统计学意义(P＜0.05);研究组出院12个月随访时一般健康状况、健康变化评分高于对照组,差异均有统计学意义(P＜0.05).结论 医共体合作下的延伸护理可提升慢性心衰患者的生活质量,值得临床推广应用.
Abstract
Objective To explore the impact of extended nursing under the cooperation of medical communities on patients with chronic heart failure(shorter form:chronic heart failure),and to provide assistance for improving their quality of life.Methods A total of 201 patients with chronic heart failure who were hospitalized and improvement discharged from Changshu First People's Hospital and its medical community hospital from January 2020 to February 2022 were selected and divided into control group with 100 cases and study group with 101 cases according to the random number table method.The control group was followed up by hospital nurses over the phone,while the study group was followed up by community medical staff under the medical community hospital.Follow up were conducted at 3,6 and 12 months after the patients'discharge.The cardiac function grading,readmission and mortality rates and quality score of life between two groups were compared.Results At the 3-month,6-month and 12-month follow-up after dischange,there were no statistically significant differences in heart function grading,readmission and mortality between the two groups(P＞0.05).At the 3-month follow-up after discharge,the scores of physical pain,general health status and health changes in the study group were higher than those in the control group,and the differences were statistically significant(P＜0.05);at the 6-month follow-up after discharge,the scores of physical pain,emotional function and health changes in the study group were higher than those in the control group,and the differences were statistically significant(P＜0.05);at the 12-month follow-up after discarge,the scores of general health status and health changes in the study group were higher than those in the control group,and the differences were statistically significant (P＜0.05).Conclusion The extended nursing under the cooperation of the medical community could improve patients with chronic heart failure quality of life,and was worthy of clinical promotion and application.
