Pathological basis study on children with congenital biliary dilatation with liver injury
Objective To explore the pathological basis of congenital biliary dilatation(CBD)with liver injury in children,in order to provide assistance for the clinical treatment of CBD with liver injury.Methods A retrospective analysis was conducted on 94 children with CBD with liver injury admitted to Tianjin Children's Hospital(Tianjin University Children's Hospital)from February 4,2022 to May 4,2023.After analyzing and organizing the clinical data of the children,they were divided into liver injury group(Group G)of 40 cases and non liver injury group(Group W)of 54 cases based on whether liver injury occurred.The clinical manifestations,cyst diameter,incidence of protein plugs/stones in cysts,incidence of obvious stenosis and atresia at the distal end of cysts,and incidence of bile stasis between two groups of pediatric patients were compared.Results The incidence of liver and spleen enlargement in Group G was 47.50%,which was higher than 18.52%in Group W;the incidence of abdominal masses in Group G was 45.00%,which was higher than that in Group W with 14.81%,and the differences were statistically significant(x2=9.047,10.464,P<0.05).There were no statistically significant differences in the incidence of jaundice,abdominal pain,fever,vomiting,and clay stool between Group G and Group W(P>0.05).The diameter of cysts in Group G was larger than that in Group W,and the incidences of protein plugs/stones,obvious stenosis and atresia at the distal end of cysts and bile stasis were higher in Group G than those in Group W,and the differences were statistically significant(P<0.05).Conclusion Children with CBD with liver injury had obvious cholestasis,hepatosplenomegaly,abdominal mass and other symptoms,the pathological basis of which was biliary obstruction cholestasis.
