目的 探讨肾小球滤过率(GFR)与突发性聋的相关性,并分析突发性聋预后的影响因素.方法 回顾性分析2021年1月-2022年10月收治且临床资料完整的突发性聋261例,比较不同临床特征的突发性聋患者估算肾小球滤过率(eGFR)水平;分析突发性聋预后的影响因素并绘制ROC曲线评价预测效能.结果 ①突发性聋患者的听力值(dB)与eGFR水平呈负相关(P=0.02);②eGFR水平在患者的性别、年龄、高血压史、糖尿病史、吸烟史及听力损失类型间比较差异具有统计学意义(P均<0.05);③eGFR水平和听力损失类型是患者预后的独立影响因素(P<0.05);④eGFR的ROC曲线下面积为0.704(95%CI:0.642~0.766),预测模型具有统计学意义(P<0.001),当约登指数为0.360时,最佳决策阈值为103.50 mL·min-1·1.73m-2.结论 ①eGFR水平与突发性聋患者的性别、年龄、高血压史、糖尿病史、吸烟史及听力损失类型有关.②eGFR水平和听力损失类型是突发性聋预后的独立影响因素.
Objective To investigate the correlation between glomerular filtration rate(GFR)and sudden deafness,and to analyze the prognostic factors of sudden deafness.Methods Clinical data of 261 patients suffering from sudden deafness who were hospitalized in our department from Jan 2021 to Oct 2022 were retrospectively analyzed,and the estimate glomerular filtration rate(eGFR)levels of the patients with different clinical characteristics were compared.The influencing factors for the prognosis of sudden deafness were analyzed and the receiver operating characteristic curve was drawn to evaluate the predictive efficiency.Results ①Hearing values(dB)of patients with sudden deafness were linearly and negatively correlated with eGFR levels(P=0.02).②The level of eGFR was associated with gender,age,hypertension,diabetes,smoking and type of hearing loss of patients with sudden deafness(all P<0.05).③The eGFR level and hearing loss type were independent influencing factors for the prognosis of sudden deafness(P<0.05).④The area under the receiver operating characteristic curve of eGFR was 0.704(95%CI:0.642~0.766),and the prediction model was statistically significant(P<0.001).When the Youden index was 0.360,the optimal decision threshold was 103.50 mL·min-1·1.73 m-2.Conclusions ①The level of eGFR is related to the patient's gender,age,history of hypertension,history of diabetes,history of smoking and type of hearing loss.②The level of eGFR and type of hearing loss are independent factors influencing the prognosis of sudden deafness.
