Etiological distribution of 1 582 patients with hyperandrogenism in outpatient
Objective To investigate the etiology and distribution characteristics of female hyperandrogenemia patients in gynecology outpatient clinics.Methods The etiology and clinical data of hyperandrogenemia patients in the gynecology outpatient clinic of Peking University People's Hospital from January 2019 to August 2019 were retrospectively analyzed.The included patients were diagnosed according to the currently recognized diagnostic criteria.Results(1)Among 1 582 patients with hyperandrogenemia,1 277 cases of polycystic ovary syndrome(80.72%),30 cases of thyroid disease(1.90%),29 cases of use of androgenic drugs(1.83%),20 cases of ovarian tumors(1.26%),14 cases of hyperprolactinemia(0.89%),adrenal factors in 3 cases(0.22%);and unknown etiology in 209 cases(13.21%).(2)The age of the patients ranged from 19 to 75 years,stratified by age etiology(excluding unknown etiology),and ranked in the top 3:<25 years old group:polycystic ovary syndrome(20.73%,328/1 582),hyperprolactinemia(0.38%,6/1 582),thyroid disease(0.32%,5/1 582);in the group of 25~35 years:polycystic ovary syndrome(51.42%,706/1 582),thyroid disease(1.17%,16/1 582)and in the group of 36~40 years:polycystic ovary syndrome(10.85%,149/1 582),thyroid disease and use of androgens both(0.32%,5/1 582),a total of 155 cases;greater than 40 years of age group:polycystic ovary syndrome(5.94%,94/1 582),thyroid disease,the use of androgens and ovarian tumors both(0.25%,4/1 582),a total of 106 cases.Conclusions The top 3 causes of hyperandrogenemia were PCOS,thyroid disease,and use of androgenic medications,adrenal disease and androgen-secreting tumors were rare;The incidence of PCOS was the highest in all age groups,and most occurred before the age of 40 years old.Further treatment should be carried out in conjunction with the etiology of the disease.
