妊娠期缺铁、镉暴露与子痫前期发病风险的相关性研究

目的 探讨孕妇体内铁(Fe)与镉(Cd)含量在子痫前期(preeclampsia,PE)发生发展中的作用。方法 筛选并收集2022年1月至2023年7月期间在云南省第一人民医院产科分娩的135例孕妇入组,按照是否发生PE,将其分为正常组(60例)和PE组(75例)。分别测定两组母血、脐带血及胎盘组织样本中的Fe与Cd含量,并进行相关性分析。结果 PE组孕妇的年龄明显偏大,产前体重和产前体质指数(BMI)均高于正常组(P<0。05)。而新生儿的出生体重、身长、5分钟Apgar评分及胎龄均显著低于正常组(P<0。05)。PE组母血Fe含量[(7。10±2。29)mg/L]明显低于正常组[(8。02±1。68)mg/L],差异有统计学意义(P<0。01),同时 PE 组中母血和胎盘的 Cd 含量[(0。08±0。07)mg/L、(0。80±1。58)mg/L]均高于正常组[(0。06±0。04)mg/L、(0。30±0。12)mg/L],差异有统计学意义(P<0。05)。相关性分析显示,正常组中母血和脐血Fe和Cd均呈明显的正相关关系(r=0。4882,P<0。0001;r=0。2943,P<0。05),PE组中母血、胎盘和脐血中Fe和Cd均显著正相关(r=0。3367,P<0。01;r=0。2910,P<0。05;r=0。2313,P<0。01)。此外,正常组与 PE 组中母血 Cd 与脐血 Cd均存在高相关性(r=0。95,P<0。0001;r=0。91,P<0。0001)。结论 孕妇体内Fe和Cd含量可能与PE有关,同时PE患者体内Cd与Fe含量之间可能存在相互作用关系。
Correlation between iron deficiency and cadmium exposure during pregnancy and the risk of preeclampsia
Objective To investigate the relationship between Fe content in pregnant women and Cd exposure in the occurrence and development of preeclampsia(PE).Methods We screened and collected 135 pregnant women who gave birth in the obstetrics department of Yunnan First People's Hospital between January 2022 and July 2023.According to the occurrence of PE,they were divided into a normal group(60 cases)and PE group(75 cases).The content of Fe and Cd in maternal blood,umbilical cord blood and placental tissue samples of the two groups were determined respectively,and correlation analysis was conducted on the relevant data.Results The age of PE women was significantly older,and the prenatal weight and prenatal body mass index(BMI)of pregnant women in PE group were higher than those in the normal group(P<0.05).The birth weight,length,Apgar 5 min score and gestational age of neonates were significantly lower than those of normal neonates(P<0.05).The content of Fe in maternal blood in PE group[(7.10±2.29)mg/L]was significantly lower than that in normal group[(8.02±1.68)mg/L],the difference was statistically significant(P<0.01),and the content of Cd in maternal blood and placenta in PE group[(0.08±0.07)mg/L,(0.80±1.58)mg/L]was higher than that of normal group[(0.06±0.04)mg/L,(0.30±0.12)mg/L],the difference was statistically significant(P<0.05).Correlation analysis showed that there was a significant positive correlation between Fe and Cd in maternal blood and cord blood in the normal group(r=0.4882,P<0.0001;r=0.2943,P<0.05),there were significant positive correlations between Fe and Cd in maternal blood,placenta and umbilical blood in PE group(r=0.3367,P<0.01;r=0.2910,P<0.05;r=0.2313,P<0.01).In addition,there was a high correlation between maternal blood Cd and cord blood Cd in both normal group and PE group(r=0.95,P<0.0001;r=0.91,P<0.0001).Conclusions The content of Fe and Cd in pregnant women may be related to PE,and there may be an interaction between Cd and Fe in PE patients.
