基于发病率及围产结局对双胎妊娠妊娠期糖尿病诊断标准的探讨

双胎妊娠妊娠期糖尿病(gestational diabetes mellitus,GDM)发病率高于单胎妊娠.单胎妊娠GDM与母儿不良围产结局密切相关,但双胎妊娠GDM对围产结局的影响观点不一.双胎妊娠GDM最佳的筛查及诊断标准、血糖管理目标等与单胎妊娠GDM并不完全相同,但目前尚无公认可行的双胎妊娠GDM的筛查和诊治策略.本研究围绕目前双胎妊娠GDM的发病情况、诊断标准及其对围产结局的影响进行综述,以便为双胎妊娠GDM临床诊断和处理提供依据.
Abstract
Gestational diabetes mellitus(GDM)is prone to occur in twin pregnancies than in singleton pregnancy.GDM is closely related to adverse maternal and neonatal outcomes in singleton pregnancy,while there are different views on the impact of GDM in twin pregnancies.The optimal screening timing,diagnostic criteria and management object of blood glucose in singleton and twin pregnancies are not identical.At present,there are no recognized or feasible screening procedures,diagnostic criteria or treatment strategy for GDM in twin pregnancies.Our study reviews the current incidence,diagnostic criteria and impact of GDM on perinatal outcomes in twin pregnancies,so as to provide evidence for clinical diagnosis and management of this disease.
