血清ACTH水平及甲状腺激素水平与肠易激综合征肠道菌群的关系

目的:分析血清促肾上腺皮质激素(ACTH)水平及甲状腺激素(TH)水平与肠易激综合征(IBS)肠道菌群的关系.方法:采用回顾性研究方法,将2021年5月至2023年5月于我院确诊为IBS患者178例设为观察组,选取216名健康者设为对照组.分别检测2组肠道内典型菌群数量与其Alpha多样性(Chao1指数和Shannon指数),血清ACTH水平变化,TH水平变化以及该两种指标与肠道典型菌群的相关性.结果:观察组患者双歧杆菌(Bifido)与乳酸杆菌(LaG-GG)均低于对照组,大肠杆菌(E.coli)与沙门氏菌(SE)均高于对照组(P<0.05).观察组患者Chao1指数、Shannon指数均低于对照组(P<0.05).观察组患者8:00时血清ACTH水平低于对照组(P<0.05),24:00时血清ACTH水平高于对照组(P<0.05),2组16:00时血清ACTH水平比较差异无统计学意义(P>0.05).观察组患者T4、FT3水平均高于对照组(P<0.05),FT4水平低于对照组(P<0.05);2组T3、TSH水平比较差异无统计学意义(P>0.05).观察组患者8:00时ACTH 以及 FT4 水平与 Bifido、LaG-GG 呈正相关,与 E.coli、SE 呈负相关(P<0.05);24:00 时 ACTH、T4、FT3 水平与Bifido、LaG-GG呈负相关,与E.coLi、SE呈正相关(P<0.05).结论:IBS患者肠道菌群多样性较低,其8:00时ACTH以及FT4水平与Bifido、LaG-GG正相关,与E.coli、SE呈负相关;24:00时ACTH、T4、FT3水平与Bifido、LaG-GG呈负相关,与E.coli、SE呈正相关.
Objective To analyze the relationship between serum levels of adrenocorticotropic hormone(ACTH)and thyroid hormone(TH)and intestinal flora in irritable bowel syndrome(IBS).Methods A retrospective study was conducted to select 178 patients diagnosed with IBS in our hospital from May 2021 to May 2023 as the observation group,and 216 healthy subjects as the control group.The number of typical bacterial flora in the intestine and its Alpha diversity(Chao1 index and Shannon index),the changes of ser-um levels of ACTH and TH,and the correlation between the two indicators and typical intestinal flora were respectively detected in the two groups.Results The bifidobacteria(Bifido)and lactobacillus(LaG-GG)in the observation group were lower than those in the control group,while the Escherichia coli(E.coli)and Salmonella(SE)in the observation group were higher than those in the control group(P<0.05).The Chaol index and Shannon index in the observation group were lower than those in the control group(P<0.05).The serum level of ACTH in the observation group was lower than that in the control group at 8:00(P<0.05),and higher than that in the control group at 24:00(P<0.05).There was no significant difference in the serum level of ACTH between the two groups at 16:00(P>0.05).The levels of T4 and FT3 in the observation group were higher than those in the control group(P<0.05),and the level of FT4 in the observation group was lower than that in the control group(P<0.05).There was no significant difference in the levels of T3 and TSH between the two groups(P>0.05).The levels of ACTH and FT4 in the observation group were positively correlated with Bifido and LaG-GG,and negative-ly correlated with E.coli and SE at 8:00(P<0.05).The levels of ACTH,T4 and FT3 were negatively correlated with Bifido and LaG-GG,and positively correlated with E.coli and SE at 24:00(P<0.05).Conclusion The diversity of intestinal flora in patients with IBS is low.The levels of ACTH and FT4 are positively correlated with Bifido and LaGGG,and negatively correlated with E.coli and SE at 8:00.The levels of ACTH,T4 and FT3 are negatively correlated with Bifido and LaG-GG,and positively correlated with E.coli and SE at 24:00.
