TROPIS改良挂线法与切开挂线术治疗高位肛瘘疗效比较

摘要
目的:观察比较经肛括约肌间切开术(TROPIS)改良挂线法与切开挂线术治疗高位肛瘘的疗效.方法:将120例高位肛瘘患者随机分为对照组和观察组,每组60例.对照组患者采用切开挂线术治疗,观察组患者采用TROPIS改良挂线法治疗,比较2组患者的疗效、术后并发症发生率以及术后复发率.结果:观察组患者总有效率率高于对照组(P＜0.05),观察组患者术后并发症发生率低于对照组(P＜0.05),观察组患者术后6、12个月复发率低于对照组(P＜0.05).结论:TROPIS改良挂线法治疗高位肛瘘疗效显著、术后并发症发生率和复发率低,值得临床推广应用.
Abstract
Objective To observe and compare the clinical efficacy of modified thread-drawing method in transanal open-ing of intersphincteric space(TROPIS)and incision thread-drawing surgery for the treat-ment of high anal fistula.Methods Total 120 patients with high anal fistula were randomly divided into control group and observation group,with 60 cases in each group.The control group was treated with inci-sion thread-drawing surgery,and the observation group was treated with modified thread-drawing method in TROPIS.The efficacy,incidence of postoperative complications and postoperative recurrence rate of the two groups were compared.Results The total effective rate of the observation group was higher than that of the control group(P＜0.05).The incidence of postoperative complications in the observation group was lower than that in the control group(P＜0.05).The postoperative recurrence rate of the ob-servation group was lower than that of the control group 6 and 12 months after operation(P＜0.05).Conclusion The modified thread-drawing method in TROPIS has significant effect on high anal fistula,low incidence of postoperative complications and recurrence rate,which is worthy of clinical promotion and application.
