Clinical Application of Standardized Management Path in Patients Undergoing Colonoscopy Through Enterostomy after Rectal Miles Operation
Objective To compare the effect of colonoscopy through enterostomy after rectal Miles oper-ation before and after the implementation of standardized management path,and to explore its application value.Methods We reviewed the data of patients who underwent colonoscopy through enterostomy af-ter rectal Miles operation before the implementation of the standardized management path(February 2022 to February 2023,the pre-implementation group for short)and after the implementation(March 2023 to March 2024,the post-implementation group for short).The clinical effects of colonoscopy before and after implementation(success rate of examination,success rate of intestinal preparation,depth of entry,time to get to ileocecal junction,examination time,enteric bubble score,and pain VAS score),psychological state of patients(SAS score and SDS score),hemodynamic indexes(heart rate,systolic blood pressure,and diastolic blood pressure),and nursing satisfaction degree were compared.Results The success rate of colonoscopy and intestinal preparation in the post-implementation group was significantly higher than that in the pre-implementation group(P<0.05).There was no significant difference in the depth of en-try between the two groups(P>0.05).The arrival time of enteroscope to ileocecal junction and exami-nation time in the post-implementation group were significantly shorter than those in the pre-implementa-tion group,and the intestinal bubble score and VAS score were significantly lower than those in the pre-implementation group(P<0.05).The heart rate and systolic blood pressure of patients in the pre-imple-mentation group during colonoscopy were significantly increased(P<0.05).There was no significant fluctuation in heart rate and systolic blood pressure(P>0.05)in the post-implementation group.The SAS score and SDS score in the post-implementation group were lower than those in the pre-implementa-tion group,and the nursing satisfaction was higher than that in the pre-implementation group(P<0.05).Conclusion The intervention of patients undergoing colonoscopy through enterostomy after rec-tal Miles operation according to the standardized management path can effectively improve the success rate of colonoscopy,improve the psychological state of patients,stabilize hemodynamic indexes,and enhance nursing satisfaction.
