Common complications and their management in the treatment of chronic osteomyelitis with Ilizarov bone transport technique
Objective:To summarize the common complications and their management methods associated with bone transport technique in the treatment of chronic osteomyelitis.Methods:A retrospective analysis was conducted on the clinical data of 15 patients with osteomyelitis treated with bone transport technique from January 2019 to October 2021.The cohort included 10 males and 5 females,aged 32 to 61 years,with an average age of(44.3±7.8)years.Radical resection of the osteomyelitic bone segment was performed,followed by reconstruction of the defective bone segment using bone transport techniques.Complications during treatment were managed accordingly.Results:All patients were followed up for 10 to 18 months,with an average follow-up duration of(13.8±1.6)months.The complications during the treatment included severe pain occurred in 5 cases due to skin cutting during bone transport,which was relieved by using analgesics and making small incision on metal skin contact surface to reduce skin tension;pin tract infections in 5 cases,which were improved by changing dressings and replacing bone penetrating elements;transporting bone segment drift in 7 cases,which was corrected by adjusting the force line during docking;and clubfoot deformity in 3 cases,which was corrected by the addition of a foot ring;leg length discrepancy in 2 cases and was resolved by bone lengthening post-docking;re-fracture after removal of the external fixator in one case and was treated with plaster fixation for bone healing,knee joint stiffness in 2 cases and was improved by loosening operation and functional exercise;soft tissue defects in one case and were managed by simultaneous soft tissue distraction and closure of the exposed wound during bone transport.Conclusions:Common complications of bone transport in the treatment of osteomyelitis include pain,pin tract infection,deformities,fractures,and soft tissue defects.These complications can be effectively managed with appropriate treatment strategies,leading to satisfactory outcomes.
