摘要

骨质疏松症(OP)以骨形成及骨吸收失衡为主要标志,对中老年人群的日常生活造成严重困扰,引发诸如骨质疏松性骨折等疾病.既往已有许多药物应用于OP的治疗,如钙剂、维生素D、二膦酸盐类、甲状旁腺素、降钙素、单克隆抗体,但因不良反应或使用周期长等,其应用受限.肠道菌群(GM)与人体健康状态息息相关,肠道微生态系统作为人体最大的生态系统,其失衡将引起多种疾病,如炎症性肠病、风湿性关节炎、糖尿病、代谢综合征等免疫或代谢相关性疾病.随着研究的深入,GM与骨代谢的相关性逐渐得到认知,其有望作为防治以OP为代表的骨代谢疾病的新兴手段.本文从骨骼运动系统、消化系统、免疫系统、内分泌系统、神经系统对骨代谢与GM相关机制的研究进展进行综述.

Abstract

Osteoporosis(OP),characterized by the imbalance of bone formation and bone resorption,significantly impacts the daily life of middle-aged and elderly people,causing diseases such as osteoporotic fractures.Numerous drugs,including calcium,vitamin D,bisphosphonates,parathyroid hormone,calcitonin,and monoclonal antibodies,have been used in the management of OP.However,their application is limited due to adverse events or extended treatment durations.The gut microbiota(GM),as the largest ecosystem within the human body,is closely related to human health,and imbalances in the intestinal microecosystem cause various diseases,such as inflammatory bowel disease,rheumatoid arthritis,diabetes mellitus,metabolic syndrome,and other disorders associated with immune or metabolic functions.As research deepens,the correlation between GM and bone metabolism has gradually been recognized in the field.GM is expected to serve as an emerging therapeutic approach for bone metabolic diseases represented by OP.This article reviews the research progress on multisystem mechanisms related to bone metabolism and GM,including the musculoskeletal,digestive,immune,endocrine,and nervous systems.
