血红蛋白与老年骨质疏松性骨折的相关性分析

摘要
目的 探讨血红蛋白水平与老年骨质疏松骨折的关系.方法 选取 2021 年 1 月至 2022 年 5 月在广州中医药大学第一附属医院骨二科住院的 50 岁以上且诊断为脊柱骨折的 324 例患者.从电子病历中收集患者骨密度、血红蛋白等医疗数据.结果 对男性骨质疏松骨折,贫血发生率明显升高,血红蛋白水平明显降低(P＜0.001);而女性未发现相关性.以贫血分组,两性的骨密度均因贫血而降低(P＜0.01).相关性分析显示,男性血红蛋白与L1～4 骨密度(r=0.466,P＜0.001)、左股骨颈骨密度(r=0.424,P＜0.001)、左髋骨密度(r=0.469,P＜0.001)相关;女性血红蛋白与L1～4 骨密度(r=0.199,P=0.002)、左股骨颈骨密度(r=0.326,P＜0.001)、左髋骨密度(r=0.341,P＜0.001)相关.二元Logistic回归发现男性中血红蛋白水平与骨质疏松症之间存在关联(OR=0.956,95%CI:0.917～0.997,P=0.035);女性未发现关联.结论 血红蛋白水平和骨密度相关,血红蛋白水平降低可能会致老年男性骨质疏松骨折高风险.
Abstract
Objective To investigate the relationship between hemoglobin(HGB)and osteoporosis fractures in elderly patients.Methods A total of 324 hospitalized patients over 50 years old,who were diagnosed with spinal fracture in the Second Department of orthopedics of Guangzhou University of Chinese Medicine from January 2021 to May 2022,were selected.Medical data such as bone mineral density(BMD)and HGB were collected from the electronic medical records.Results In men,anemia incidence increased significantly and HGB decreased significantly(P＜0.001).No association was found in women.Classified by anemia,BMD decreased in both genders due to anemia(P＜0.01).In men,HGB was associated with L1-4 BMD(r=0.466,P＜0.001),left femoral neck BMD(r=0.424,P＜0.001),and left total hip BMD(r=0.469,P＜0.001).In women,HGB was associated with L1-4 BMD(r=0.199,P=0.002),left femoral neck BMD(r=0.326,P＜0.001),and left total hip BMD(r= 0.341,P＜0.001).Binary logistic regression found an association between HGB and osteoporosis in men(OR=0.956,95%CI:0.917-0.997,P=0.035).No association was found in women.Conclusion HGB is related to BMD.Lower HGB may cause a high risk of osteoporosis fracture in elderly men.
