破骨细胞介导骨性疼痛的研究进展

摘要
骨骼具有高度的神经支配,随着骨生物学研究的进展,感觉神经和骨重塑之间的偶联机制逐渐受到重视.由感觉神经元介导的骨性疼痛在骨病的发展、诊断、治疗和疗效评估中具有重要意义.目前引起骨性疼痛的具体病理机制仍未被完全阐明,然而研究表明主导骨吸收的破骨细胞在伤害感受性骨痛和神经病理性骨痛中发挥重要作用.因此本文综述了近年来的相关研究,探讨了破骨细胞介导骨性疼痛的相关机制,包括酸性微环境、炎性环境、破骨细胞分泌的相关疼痛因子刺激感觉神经的伤害感受器以及破骨细胞导致的感觉神经病理性改变,同时探讨了破骨细胞在不同骨病中介导骨性疼痛的作用.旨在总结破骨细胞介导骨性疼痛的相关研究进展,以期为骨性疼痛的治疗提供新的思路和靶点.
Abstract
The skeletal system,highly innervated,is gaining increased attention in bone biology research,focusing on the interplay between sensory neurons and bone remodeling.Sensory neurons-mediated bone pain emerges as a pivotal factor in the development,diagnosis,treatment,and efficacy evaluation of bone diseases.While the precise pathomechanisms underlying bone pain remain partially elucidated,current studies highlight the substantial role of osteoclasts,the primary mediators of bone resorption,in both nociceptive and neuropathic bone pain.This review comprehensively summarizes recent relevant researches,delving into the mechanisms through which osteoclasts contribute to bone pain.Key aspects explored include the acidic microenvironment,inflammatory conditions pain-inducing factors secreted by osteoclasts that stimulate nociceptors,as well as the neuropathic changes induced by osteoclasts on sensory neurons.Furthermore,the review examines the involvement of osteoclasts in mediating bone pain across various bone diseases.The overarching goal of this article is to provide a consolidated overview of advancements in understanding osteoclast-mediated bone pain,offering fresh perspectives and potential targets for the treatment of bone pain.
