咖啡摄入增加骨关节炎风险:一项孟德尔随机化研究

摘要
目的 使用孟德尔随机化(Mendelian Randomization,MR)研究探索咖啡摄入与骨关节炎(osteoarthritis,OA)之间的因果关系.方法 使用公开发布的全基因组关联研究(genome-wide association study,GWAS)汇总数据进行了孟德尔随机化分析,采用逆方差加权法、加权中位数法、简单中位数法、加权中值方法和MR-Egger回归法估计咖啡摄入与OA风险的因果关系,并进行异质性检验、敏感性分析、多效性检验验证结果稳定性.使用不同种类咖啡摄入进一步验证其对OA风险的影响.结果 MR分析表明,咖啡摄入对OA产生不利影响,其中自我报告OA:OR(95%CI)为 2.16(1.65~2.82);髋关节或膝关节OA:OR(95%CI)为 2.22(1.74~2.84);髋关节OA:OR(95%CI)为 2.17(1.45~3.24);膝关节OA:OR(95%CI)为 2.37(1.83~3.07).敏感性分析显示结果比较稳定,多效性检验显示不存在水平多效性.进一步验证发现咖啡摄入确实会导致OA风险增加,但不同咖啡类型会对OA产生不同影响.结论 本研究结果表明,咖啡摄入增加了骨关节炎患病风险.
Abstract
Objective To analyze the relationship between coffee intake and Osteoarthritis(OA)using mendelian randomization.Methods The IEU Open GWAS project(https://gwas.mrcieu.ac.uk/)was the source of exposure and outcome datasets.The exposure datasets included coffee intake,while the outcome datasets included self-report osteoarthritis,knee osteoarthritis,hip osteoarthritis and osteoarthritis of the knee or hip.The weighted median,MR-Egger,Inverse Variance Weighted,Simple Mode,and Weighted Mode were used as the method of mendelian randomization analysis.We also performed the Heterogeneity,pleiotropic and sensitivity analysis.Using different types of coffee intake to further validation the coffee intake effect on OA risk.Results MR analysis shows that coffee intake has an adverse effect on OA,with self-reported OA:OR(95%CI)of 2.16(1.65-2.82);Hip or knee joint OA:OR(95%CI)was2.22(1.74-2.84);Hip joint OA:OR(95%CI)was 2.17(1.45-3.24);Knee joint OA:OR(95%CI)was 2.37(1.83-3.07).The sensitivity analysis of the leave-one-out method showed that the result were relatively stable,and there was no horizontal pleiotropy in the pleiotropy test.The same conclusion was reached in the external validation.Conclusion The result showed that coffee intake increases the risk of osteoarthritis.
