剖宫产后再次妊娠孕妇分娩决策冲突的质性研究

目的:分析剖宫产后再次妊娠孕妇的分娩决策行为冲突,为制定决策支持策略提供依据.方法:通过目的抽样法选取13名剖宫产术后再次妊娠孕妇为研究对象,采用描述性质性研究方法对其进行半结构式访谈,应用内容分析法进行分析.结果:剖宫产后妊娠孕妇的分娩决策行为冲突包括决策认知受限是行为冲突前因(分娩决策复杂、信息限制)、知情决策不足是行为过程冲突诱因(价值观冲突、多元决策主体冲突)、决策冲突行为及体验(决策行为表现、决策情感体验).结论:剖宫产后妊娠孕妇在分娩决策过程中受多重因素影响,护士应制定适宜的决策支持策略,以期尽可能降低其决策冲突,提升决策质量,以降低盲目再次剖宫产率.
Decision conflict of delivery modes among pregnant women after cesarean section:a qualitative study
Objective:To analyze the decision conflict as to delivery modes of pregnant women after cesarean section,and provide basis and assistance for future decision-making.Methods:The descriptive qualitative research design was used to conduct in-depth semi-structured interviews with 13 pregnant women who had received cesarean section by purposive sampling.Content analysis was adopted to analyze the obtained data.Results:Conflicts in deciding delivery modes of pregnant women after cesarean section included limited information for decision-making as the antecedent of their behavioral conflict(complex process of decision-making,information restrictions),inadequate informed decision-making as the cause of behavioral process conflict(conflict of values,conflict among multiple decision-making subjects),and experience of decision-making conflict(decision-making behavior performance,emotional experience of decision-making).Conclusion:Pregnant women after cesarean section are affected by multiple factors when determining the modes of delivery.Nurses should take appropriate measures to assist patients to make proper decisions,so as to avoid conflict in decision-making,improve the quality of patients'decision,and reduce the rate of repeated cesarean section.
