摘要
目的:检索并汇总慢性肾脏病青少年向成人医疗过渡管理相关证据,为提升我国慢性肾脏病青少年过渡期医疗护理服务质量提供参考.方法:依据"6S"金字塔证据模型,系统检索相关网站与数据库获取慢性肾脏病青少年向成人过渡管理的相关文献,检索时限为建库至2024年6月3日,由经过系统循证护理培训的研究者对纳人文献进行方法学质量评价和资料提取后汇总证据.结果:共纳入10篇文献,包括4篇指南、4篇专家共识和2篇系统评价,围绕组建团队、过渡准备、过渡期照护、融入成人医疗4个类别形成23条证据.结论:本研究总结的有关慢性肾脏病青少年向成人医疗过渡管理的证据,可为今后制定我国慢性肾脏病青少年向成人医疗过渡管理方案提供理论基础.
Abstract
Objective:To extract and summarize the evidence related to the transition management of adult health care for adolescents with Chronic Kidney Disease,and provide a reference for promoting high-quality transitional care for adolescents with Chronic Kidney Disease in China.Methods:According to the"6S"pyramid evidence model,we systematically searched articles related to the transition management of adolescents with Chronic Kidney Disease to adults from relevant websites and databases.Time limit for the search from the establishment of the database to June 3,2024.Researchers trained in evidence-based nursing evaluated the methodological quality of the included literature and collected the evidence after data extraction.Results:A total of 10 articles were included,including 4 guidelines,4 expert consensus and 2 systematic reviews.There were 23 pieces of evidence that focus on four key aspects:team building,transition preparation,transition care and integration into adult healthcare.Conclusion:This study summarizes the evidence on the transition management for adolescents with Chronic Kidney Disease,which can provide a theoretical basis for the development of medical transition management programs for adolescents with Chronic Kidney Disease in China.
