Benefits of Joint Application of Multifunctional Radiofrequency Plasma Sur-gery and CO2 Laser in Improving Voice,Swallowing and Laryngeal Functions in Treatment of Early Supraglottic Laryngeal Cancer
Objective To analyze the benefits of multifunctional radiofrequency plasma surgery combined with carbon dioxide(CO2)laser in improving voice,swallowing and laryngeal functions in treating early supraglottic laryngeal cancer.Methods Totally 76 patients with early supraglottic laryngeal cancer treated over the period from Sep.2018 to Nov.2020 were selected and divided into two groups by treating methods:a control group consisting of 38 patients given multifunctional radiofrequency plasma surgery,and an experimental group consisting of 38 patients treated jointly with multifunctional radiofrequency plasma surgery and CO2 laser(power:1-3 W,spot diameter:0.5-1.0 mm,pulse width:0.1-0.2 ms).The surgical time,intraoperative bleeding volume and length of hospital stay were compared between the two groups,and the pain situation of the patients in the two groups was recorded with visual analog scale(VAS)48 hours after the surgery.The acoustic changes to the fundamental frequency,fundamental frequency perturbation,amplitude perturbation and harmonic to noise ratio of the patients in the two groups were recorded before and 1 year after the surgery respectively.The swallowing function score and laryngeal function preservation rate were compared.In addition,the incidence of complications and 3-year postoperative recurrence rate in both groups were observed.Results(1)Compared with the control group,the experimental group had shorter surgical time and hospital stay and smaller intraoperative bleeding volume and 48h postoperative VAS score(P<0.05).(2)One year after the surgery,the fundamental frequency,fundamental frequency perturbation and amplitude perturbation of the patients in both groups were lower than those before the surgery while the harmonic to noise ratio being higher.The fundamental frequency,fundamental frequency perturbation and amplitude perturbation indicators in the experimental group were lower than those in the control group while the harmonic to noise ratio being higher in the experimental group(P<0.05).(3)One year after the surgery,the swallowing function scores in both groups were higher than those before the surgery,and the scores were higher in the experimental group than in the control group(P<0.05).The postoperative laryngeal function preservation rate was 94.7%in the experimental group and 98.0%in the control group.The postoperative laryngeal function preservation rate in the experimental group was significantly higher than that in the control group(P<0.05).There was no statistically significant difference in 3-year local recurrence rate between the two groups(P>0.05).(4)The incidence of complications was 7.89%in the experimental group and 26.3%in the control group,and the difference was statistically significantly(P<0.05).Conclusions The combination of multifunctional radiofrequency plasma surgery and carbon dioxide(CO2)laser treatment for early supraglottic laryngeal cancer will effectively reduce surgical time,bleeding and pain,improve swallowing and laryngeal functions and reduce complications.It is hence worthy of widespread promotion in the clinical treatment of early supraglottic laryngeal cancer.
