Study Based on NLRP3 Inflammasome:Effect and Mechanism of Nd:YAP Laser in Treating Periodontitis
Objective To investigate the effect and possible mechanism of Nd:YAP laser in the treatment of periodontitis based on NLRP3 inflammasome.Methods Totally 140 patients with periodontitis treated from Sep.2022 to 2023 Sep.were selected and divided randomly into a control group and Nd:YAP laser group,70 patients in each group.The patients in both groups were firstly treated with ultrasonic supringival cleaning and subgingival curetting.Then the patients in the Nd:YAP laser group were given 3%hydrogen peroxide solution for rinsing periodontal bags and Nd:YAP laser intraperiodontal bag irradiation while the patients in control group being given only 3%hydrogen peroxide solution for rinsing periodontal bags.The periodontal health,level of inflammatory factors in gingival crevicular fluid and short-term curative effect were compared between the two groups,and the influence of Nd:YAP laser treatment on NLRP3 inflammasome was analyzed.Results For both groups,the PD and GI were lower 6 and 12 weeks after the treatment than before the treatment(P<0.05).The PD and GI in the Nd:YAP laser group were lower than those in the control group(P<0.05).The levels of IL-6,TNF-α and CRP in gingival crevicular fluid were lower in the Nd:YAP laser group than in the control group(P<0.05).Twelve weeks after the treatment,Nd:YAP laser group had higher efficacy class and total effective rate than the control group(P<0.05).The expression levels of NLRP3 mRNA,ASC mRNA and caspase-1 mRNA in gingival crevicular fluid in the Nd:YAP laser group were lower than those in the control group 6 and 12 weeks after the treatment(P<0.05).Conclusions Addition of Nd:YAP laser to the overall treatment of periodontitis patients will further improve the periodontal health status of patients,reduce inflammation and enhance short-term curative effect.The realization of this effect may be related to the effect of Nd:YAP laser on inhibiting the activation of NLRP3 inflammasome.
