Join Effect of Hyaluronic Acid Dressing and Superpulse Fractional CO2 Laser on Subcutaneous Tissue Oxygen Saturation in Treatment of Depressed Acne Scars
Objective To explore the joint effect of hyaluronic acid dressing and superpulse fractional CO2 laser on subcutaneous tissue oxygen saturation in the treatment of depressed acne scars. Methods Totally 120 patients with mild to moderate acne scars treated between Mar.2021 to Mar.2023 were selected and divided randomly into a control group and an observation group,60 patients in each group. While the patients in the control group being given only CO2 laser treatment, the patients in the observation group were applied hyaluronic acid dressing in addition. Moreover, propensity scores were matched to adjust for balance in the observation group.Altogether, three CO2 laser treatments(energy:10-15 mJ,spot size:15 cm×15 cm)were given at an interval of one month. The scab formation, scab shedding and healing time in the two groups were compared.ECCA scores were used to evaluate the improvement of scars before the treatment and 1 month and 3 months after the treatment respectively.VAS scores were used to determine the patients'pain situation. The microcirculation structure(including HbO2 and reduced Hb)of subcutaneous tissues and skin barrier function indicators(including pH value, epidermal water loss and skin moisture content)were detected. Results One month after the third treatment, the total clinical effective rates were 94.6%(52/55)in the observation group and 78.2%(43/55)in the control group respectively.During the treatment, the time for scab formation, shedding and healing were apparently shorter in the observation group than in the control group(P<0.05).One month after the first treatment or one month after the third treatment, the ECCA scores of both groups were much lower than those before the treatment(P<0.05),and the scores were lower in the observation group than in the control group(P<0.05).In both groups, the VAS scores decreased gradually one month after the first treatment or one month after the third treatment(P<0.05),and the scores were much lower in the observation group than in the control group(P<0.05).One month after the first treatment or one month after the third treatment, the levels of HbO2 and reduced Hb in both groups of patients were lower than those before the treatment(P<0.05),and the levels were much lower in the observation group than in the control group(P<0.05).One month after the first treatment or one month after the third treatment, the skin pH and epidermal moisture loss in both groups were lower than those before the treatment, and the values were smaller in the observation group than in the control group(P<0.05).One month after the first treatment and one month after the third treatment, the skin moisture content of both groups of patients was higher than that before treatment, and the level was higher in the observation group than in the control group(P<0.05).The incidence of adverse reactions in the observation group was lower than that in the control group(P<0.05).Conclusions In the clinical treatment of concave acne scars, the combination of hyaluronic acid dressing and CO2 laser will significantly enhance the clinical efficacy and greatly improve the scar situation and subcutaneous tissue oxygen saturation of the patients, which makes it worthy of broad promotion and application.
