Pathogen spectrum of acute meningitis and encephalitis syndrome cases in Jinan city,2017-2021
Objective To determine the spectrum of pathogens of acute meningitis and encephalitis syndrome(AMES)cases in Jinan city.Methods We collected serum and/or cerebrospinal fluid samples of AMES cases in seven sentinel hospitals of Jinan during 2017-2021.We tested for Japanese encephalitis virus(JEV),enterovirus(EV),mumps virus(MuV),and herpes simplex virus(HSV)IgM antibodies and Neisseria meningitides(Nm),Streptococcus pneumonia(Spn),and Haemophilus influenza type b(Hib)antigens to determine positivity rates.Results Prevalences of JEV,EV,MuV,and HSV IgM antibodies were 3.15%(82/2 604),13.62%(298/2 188),10.92%(239/2 188),and 4.94%(108/2 188),respectively,with the highest age-specific positivity rates being 7.61%in ≥40-year-olds,30.09%in 5-9-year-olds,16.52%in 5-9-year-olds,and 7.16%in ≥40-year-olds.Positivity rates of Nm,Spn,and Hib antigens were 2.07%(50/2 420),1.03%(25/2 420),and 0.08%(2/2 420),with the highest age-specific positivity rates being 4.65%in 25-29-year-olds,2.56%in 20-24-year-olds,and 0.17%in ≥40-year-olds.Group B Nm cases accounted for 44%(22/50)of Nm antigen positive cases.Conclusions The dominant AMES pathogens in Jinan during 2017-2021 were EV,MuV,HSV,and JEV.We should strengthen childhood EV and MuV vaccination,conduct JEV vaccination of ≥40-year-olds,and accelerate group B Nm vaccine research,development,and use.
