目的 了解云南省两种预防接种服务模式时期接种单位和县级疾病预防控制中心(Center for disease control and prevention,CDC)免疫规划(Expanded Program on Immunization,EPI)工作成本.方法 采用分层随机抽样方法在云南省选择3个县9个乡镇收集2018-2022年接种单位和县级CDC的EPI工作成本,分析村级和乡镇集中接种时期年均总成本、按出生儿童数计算的人均成本和按EPI疫苗接种剂次计算的剂均成本.结果 村级、乡镇集中接种模式时期接种单位的EPI工作年均总成本分别为207.40万元、120.00万元,人均成本分别为452.86元/人、399.39元/人,剂均成本分别为24.96元/剂、21.10元/剂;两时期县级CDC的EPI工作年均总成本分别为347.22万元、284.16万元,人均成本分别为206.36元/人、190.41元/人,剂均成本分别为10.37元/剂、9.05元/剂.结论 调查地区乡镇集中接种模式与村级集中接种模式相比可减少EPI工作成本.云南省各地区选择预防接种服务模式仍需因地制宜,合理分配公共卫生资源.
Implementation costs of the Expanded Program on Immunization of vaccination units and county-level centers for disease control and prevention during periods with two immunization service models:a survey in three counties of Yunnan province
Objective To explore implementation costs of the Expanded Program on Immunization(EPI)of vaccination units and county-level centers for disease control and prevention(CDCs)during periods with two immunization service models in Yunnan province.Methods We used a stratified random sampling method to select three counties and nine townships in Yunnan and collected data on EPI implementation costs of vaccination units and county-level CDCs from 2018 to 2022.We analyzed total average annual cost,average cost per live birth,and average cost per EPI vaccine dose administered during periods of village-and township-concentrated vaccination.Results Total average annual EPI costs of vaccination units during periods of village-and township-concentrated vaccination were 2.074 million and 1.200 million yuan,respectively,for average costs of 452.86 and 399.39 yuan per live birth or 24.96 and 21.10 yuan per dose.Total average annual EPI costs of county-level CDCs during the two periods were 3.472 2 million and 2.841 6 million yuan,for average costs of 206.36 and 190.41 yuan per live birth or 10.37 and 9.05 yuan per dose.Conclusions A township-concentrated vaccination model could reduce EPI implementation cost of vaccination units compared with a village-concentrated vaccination model.The choice of immunization service models in different areas of Yunnan should be adapted to local conditions for a more effective allocation of public health resources.
