目的 了解四川省乙型肝炎(乙肝)病毒(Hepatitis B virus,HBV)基因型和S基因变异.方法 收集2022-2023年四川省11个监测点乙肝病例的乙肝表面抗原阳性血清样本,提取病毒核酸,扩增和测定S基因序列,构建系统进化树,分析HBV基因型和α抗原决定簇氨基酸变异.结果 共获得115株HBV毒株基因序列,其中B基因型50株、C基因型58株和D基因型7株,核苷酸序列同源性分别为94.8%-100%、95.0%-100%和96.0%-99.3%,氨基酸序列同源性分别为92.2%-100%、91.0%-100%和96.0%-99.0%.28株(24.35%)HBV毒株出现α抗原决定簇氨基酸变异,主要分布在该区域的第一环内和126、127、129、131、133、134、140位点;1株在123与124位点之间插入了氨基酸K和A,1株172位点的氨基酸变异为终止密码子,各1株出现新的糖基化位点N129和N131.结论 四川省HBV优势流行株为B和C基因型,且出现一定比例的HBV抗原性改变.
Genotypes and S gene mutations of hepatitis B viruses isolated from hepatitis B cases during 2022-2023 in monitoring sites of Sichuan province
Objective To identify genotypes and S gene mutations of hepatitis B virus(HBV)isolates in Sichuan province.Methods We collected hepatitis B surface antigen positive serum samples from hepatitis B cases during 2022-2023 in 11 monitoring sites of Sichuan,extracted HBV nucleic acid,amplified S gene sequences,and constructed a phylogenetic tree.We analyzed HBV genotypes and amino acid mutations in α antigenic determinants.Results We obtained gene sequences of 115 HBV strains,in which 50,58,and 7 strains were B,C,and D genotype,respectively,with nucleotide homologies of 94.8%-100%,95.0%-100%,and 96.0%-99.3%and amino acid homologies of 92.2%-100%,91.0%-100%,and 96.0%-99.0%.Twenty-eight HBV strains(24.35%)had amino acid mutations in αantigenic determinants.Most mutations were in the first ring of α antigenic determinants at sites 126,127,129,131,133,134,and 140.One strain had amino acids K and A inserted between sites 123 and 124.One strain had a stop codon amino acid mutation at site 172.New glycosylation sites N129 and N131 were found in another strain.Conclusion The HBV strains circulating in Sichuan were mostly B and C genotype,with some antigenic changes.
