目的 分析中国无细胞百白破联合疫苗(Diphtheria,tetanus and acellular pertussis combined vaccine,DTaP)疑似预防接种异常反应(Adverse event following immunization,AEFI)发生特征.方法 通过中国疾病预防控制信息系统收集2018-2022年报告的DTaP常规接种剂次数和AEFI个案,描述性分析AEF1估算报告发生率.结果 中国2018-2022年共报告DTaP AEFI 292 642例,总报告发生率为108.07/10万剂;其中一般反应、异常反应分别为282 561例(96.56％)、8 459例(2.89％),报告发生率分别为104.35/10万剂、3.12/10万剂.报告例数(发生率)居前五位的异常反应依次为过敏性皮疹7 006例(2.59/10万剂)、血管性水肿462例(0.17/10万剂)、血小板减少性紫癜症142例(0.05/10万剂)、热性惊厥131例(0.05/10万剂)、无菌性脓肿129例(0.05/10万剂).结论 中国2018-2022年DTaP AEFI以一般反应为主,异常反应极其罕见.
Abstract
Objective To analyze characteristics of adverse events following immunization(AEFI)with diphtheria,tetanus and acellular pertussis combined vaccine(DTaP).Methods We collected data on doses administered and DTaP AEFI cases reported after routine vaccination during 2018-2022 to the China Disease Control and Prevention Information System.We conducted a descriptive analysis of estimated incidence of AEFIs.Results There were 292 642 DTaP AEFIs reported in China during the study period,for an overall incidence of 108.07 per 100 000 doses administered.Among AEFIs,282 561(96.56％)and 8 459(2.89％)cases were common and rare vaccine reactions,respectively,yielding reported incidences of 104.35 and 3.12 per 100 000 doses administered.The top five rare reactions in reported cases(incidences per 100 000 doses)were allergic rash(7 006 cases,2.59),angioedema(462 cases,0.17),thrombocytopenic purpura(142 cases,0.05),febrile convulsion(131 cases,0.05),and aseptic abscess(129 cases,0.05).Conclusion Most reported DTaP AEFIs were common vaccine reactions during 2018-2022 in China;serious vaccine reactions were extremely rare.
