Teach-back健康教育模式对妇科恶性肿瘤患者术后早期活动及心理状态的影响

目的:观察Teach-back健康教育模式对妇科恶性肿瘤患者术后早期活动及心理状态的影响.方法:选取2022年11月-2023年6月本院收治的妇科恶性肿瘤手术患者80例,随机数字表法分为Teach-back组及常规组各40例,均采取常规围术期早期活动宣教,Teach-back组同时采取Teach-back健康教育模式.比较两组术后3 d内活动恢复情况、活动依从性评分、早期康复情况及干预前后简易焦虑抑郁评估量表(PHQ-4)、一般自我效能感量表(GSES)评分.结果:Teach-back组术后活动恢复情况良好占比(63.2%)优于常规组(18.4%),单因素logistic回归分析显示,Teach-back健康教育模式与术后活动恢复情况有关(OR=0.186、95%CI 0.109~0.317)(P<0.05);Teach-back组术后3 d活动依从性评分(5.62±0.71分)高于常规组(5.00±0.75分),首次排气时间(20.18±5.42h)、首次离床活动时间(18.37±4.04h)均短于常规组(30.95±8.17h、25.28±4.83h);两组干预后PHQ-4评分均降低且Teach-back组(4.89±0.55分)低于常规组(5.95±0.62分),GSES评分提高且Teach-back组(34.13±2.12分)高于常规组(31.10±2.59分)(均P<0.05).结论:应用Teach-back健康教育模式可增强妇科恶性肿瘤患者术后自我效能感,缓解焦虑、抑郁情绪,提升术后早期活动依从性,加速术后康复进程.
Effects of Teach-back health education model for treating patients after gynecological malignant tumor surgery on their ear-ly postoperative activity and psychological state
Objective:To observe the effects of Teach-back health education model for treating patients after gynecolog-ical malignant tumor surgery on their early postoperative activity and psychological state.Methods:A total of 80 pa-tients with gynecological malignant tumor who wanted surgery were selected and were divided into two groups(40 ca-ses in each group)according to the random number table method from November 2022 to June 2023.The patients in control group were given the routine perioperative early activity education,while the patients in study group were given Teach-back health education mode combined with the routine perioperative early activity education.The activity recov-ery,the activity compliance score,the early rehabilitation,and the scores of simple anxiety and depression assessment scale(PHQ-4)and general self-efficacy scale(GSES)before and after intervention of the patients within 3 days after surgery were compared between the two groups.Results:The proportion of the good postoperative activity recovery(63.2%)of the patients in the study group was significantly better than that(18.4%)of the patients in the control group.Univariate logistic regression analysis showed that Teach-back health education mode of the patients was associ-ated with their postoperative activity recovery(OR=0.186,95%CI 0.109-0.317).The activity compliance score(5.62 ±0.71 points)of the patients in the study group within 3 days after surgery was significantly higher than that(5.00± 0.75 points)of the patients in the control group.The first exhaust time(20.18±5.42h)and the time of the first getting off the bed(18.37±4.04h)of the patients in the study group were significantly shorter than those(30.95±8.17h and 25.28±4.83h)of the patients in the control group.After the intervention,the PHQ-4 score of the patients in the two groups had decreased significantly,and which(4.89±0.55 points)of the patients in the study group was significantly lower than that(5.95±0.62 points)of the patients in the control group.The GSES score of the patients in the two groups had increased significantly,and which(34.13±2.12 points)of the patients in the study group was significantly higher than that(31.10±2.59 points)of the patients in the control group(all P<0.05).Conclusion:The application of Teach-back health education model for treating the patients after gynecological malignant tumor surgery can enhance their postoperative self-efficacy,relieve their anxiety and depression,improve their compliance of early postoperative activity and accelerate their postoperative rehabilitation process.
