IVF-ET胚胎培养液中sHLA-G、MMP-9与胚胎质量及临床妊娠关系

目的:分析体外受精-胚胎移植(IVF-ET)胚胎培养液中可溶性人类白细胞抗原G(sHLA-G)、基质金属蛋白酶(MMP-9)与胚胎质量、临床妊娠的关系.方法:收集2017年1月 2023年1月在本院生殖中心接受IVF-ET治疗的女性患者107例临床资料及标本,新鲜胚胎移植后收集胚胎培养液,分别取未移植的正常受精胚胎中优质胚胎、中等胚胎、劣质胚胎对应胚胎培养液,酶联免疫吸附实验检测胚胎培养液中sHLA-G、MMP-9水平,比较不同胚胎质量对应的胚胎培养液sHLA-G、MMP-9水平及其相关性;记录IVF-ET患者临床妊娠情况并分为临床妊娠组、未妊娠组,比较两组胚胎培养液中的sHLA-G、MMP-9水平及其他临床资料,logistic回归分析胚胎培养液中sHLA-G、MMP-9水平与临床妊娠关系.结果:107例未移植的正常受精胚胎中共收集127份胚胎培养液标本,其中优质胚胎培养液44份、中等胚胎培养液52份、劣质胚胎培养液30份,其胚胎培养液sHLA-G、MMP-9水平逐渐降低,胚胎培养液sHLA-G、MMP-9水平与胚胎质量正相关(均P<0.01)o 107例IVF-ET获得临床妊娠69例,临床妊娠率64.6%,未妊娠组胚胎培养液 sHLA-G(8.11±1.63 ng/ml)、MMP-9 水平(0.45±0.17 ng/ml)均低于临床妊娠组(10.63±2.41 ng/ml、0.61±0.12 ng/ml),第三日卵裂球数(6.0±1.8个)、优质胚胎率(45.6±12.1)%低于临床妊娠组(8.9±1.8个、64.8%±15.9%)(均P<0.05);logistic回归分析,sHLA-G、MMP-9、第三日卵裂球数、优质胚胎率等均是影响IVF-ET患者临床妊娠因素.结论:1VF-ET患者胚胎培养液中sHLA-G、MMP-9水平与胚胎质量正相关,sHLA-G、MMP-9、第三日卵裂球数、优质胚胎率等降低均是IVF-ET患者临床妊娠影响因素.
Correlation between the levels of the soluble human leukocyte antigen G and the matrix metalloproteinase in the embryo culture medium of women after in vitro fertilization embryo transfer and their embryo quality and clinical pregnancy
Objective:To analyze the correlation between the levels of the soluble human leukocyte antigen G(sHLA-G)and the matrix metalloproteinase(MMP-9)in the embryo culture medium of women after in vitro fertilization em-bryo transfer(IVF-ET)and their embryo quality and clinical pregnancy.Methods:The clinical data,the samples,and the embryo culture medium after fresh embryo transfer of 107 women who had received IVF-ET treatment in the re-productive center from January 2017 to January 2023 were collected.The embryo culture mediums were collected from the high-quality embryos,medium embryos and inferior embryos in the normal fertilized embryos without transplanta-tion.The enzyme-linked immunosorbent assay was used to detect the levels of sHLA-G and MMP-9 in the embryo cul-ture medium.The levels of the sHLA-G and MMP-9 in the embryo culture medium were compared among the women with different embryo quality.The correlation between the levels of the sHLA-G and MMP-9 in the embryo culture medium and their embryo quality was analyzed.The clinical pregnancy situations of the women with IVF-ET were re-corded,and the women were divided into group A(women with clinical pregnancy)and group B(women without clinical pregnancy).The levels of sHLA-G and MMP-9 in the embryo culture medium and the other clinical data of the women were compared between the two groups.Logistic regression analysis was used to analyze the correlation be-tween the levels of sHLA-G and MMP-9 in the embryo culture medium of the women and their clinical pregnancy.Re-sults:A total of 127 samples of the embryo culture medium were collected from 107 normal fertilized embryos without transfer,including 44 embryo culture mediums of the high-quality embryo,52 embryo culture mediums of the medium-quality embryo and 30 embryo culture mediums of the poor-quality embryo.The levels of sHLA-G and MMP-9 in the embryo culture medium of the high-quality embryo,the medium-quality embryo and the poor-quality embryo had de-creased gradually.The levels of sHLA-G and MMP-9 in the embryo culture mediums of the women were positively correlated with their embryo quality(all P<0.01).In 107 women with IVF-ET,there were 69 cases with clinical pregnancy,and with the clinical pregnancy rate of 64.6%.The levels of sHLA-G(8.11±1.63 ng/ml)and MMP-9(0.45±0.17 ng/ml)in the embryo culture mediums of the women in group B were significantly lower than those(10.63±2.41 ng/ml and 0.61±0.12 ng/ml)of the women in group A.The blastomere number(6.0±1.8)and the high-quality embryo rate(45.6±12.1%)of the women in group B on the third day after IVF-ET were significantly lower than those(8.9±1.8 and 64.8%±15.9%)of the women in group A(all P<0.05).Logistic regression analysis showed that the decrease of the levels of sHLA-G and MMP-9,the number of blastere on the third day after IVF-ET and the rate of the high-quality embryos of the women with IVF-ET were the factors affecting their clinical pregnancy.Conclu-sion:The levels of sHLA-G and MMP-9 in the embryo culture medium of the women with IVF-ET are positively cor-related with their embryo quality.The decrease of the levels of sHLA-G and MMP-9,the number of blastomere on the third day after IVF-ET and the rate of the high-quality embryos of the women with IVF-ET are the factors affecting their clinical pregnancy.
