妊娠期甲状腺功能减退患者Th1、Th2相关细胞因子水平与不良妊娠结局关系

目的:探讨妊娠期亚临床甲状腺功能减退(SCH)患者血清Th1、Th2相关细胞因子与不良妊娠结局关系.方法:回顾性收集2021年1月—2023年5月本院诊治的妊娠期SCH患者70例临床资料,根据妊娠结局分为结局良好组(n=42)和不良组(n=28),分析两组基本信息,比较收入院时患者Th1相关细胞因子[白细胞介素-2(IL-2)、肿瘤坏死因子-α(TNF-α)、γ-干扰素(IFN-γ)]、Th2相关细胞因子(IL-4、IL-10、IL-6)水平,对患者各因子指标与不良妊娠结局行Point-biserial相关性分析、多因素logistic回归分析.结果:SCH患者妊娠结局不良组血清IL-2、TNF-α、IFN-γ、IL-6高于结局良好组,IL-4、IL-10水平低于结局良好组(P<0.05).Point-biserial相关性分析,IL-2、IFN-γ、TNF-α、IL-6水平与不良妊娠结局正相关,IL-10、IL-4水平与不良妊娠结局负相关(均P<0.001).logistic回归分析显示,妊娠期SCH患者血清IL-2、TNF-α、IFN-γ、IL-6及促甲状腺激素升高,IL-4、IL-10降低是不良妊娠结局危险因素.结论:妊娠期SCH患者血清IL-2、TNF-α、IFN-γ、IL-6水平升高,IL-4、IL-10水平降低,水平异常与不良妊娠结局存在相关性.
Correlation between the Th1 and Th2-related cytokines levels of pregnant women with subclinical hypothyroidism and their adverse pregnancy outcomes
Objective:To investigate the correlation between the Th1 and Th2-related cytokines levels of pregnant women with subclinical hypothyroidism(SCH)and their adverse pregnancy outcomes.Methods:The clinical data of 70 pregnant women with SCH admitted to the hospital from January 2021 to May 2023 were collected retrospectively.All these women were divided into group A(42 women with normal pregnancy outcomes)and group B(28 women with ad-verse pregnancy outcomes)according to their pregnancy outcomes.The basic information of the women in the two groups was analyzed.The levels of Th1-related cytokines,such as interleukin(IL)-2,tumor necrosis factor-α(TNF-α),γ-interferon(IFN-γ),and Th2-related cytokines,such as IL-4,IL-10 and IL-6,of the women were compared between the two groups.Point-biserial correlation analysis and multiple logistic regression analysis were conducted to analyze the correlation between the Th1 and Th2-related cytokines levels of the women with SCH and their adverse pregnancy outcomes.Results:The levels of serum IL-2,TNF-α,IFN-γ and IL-6 of the women in group B were significantly higher than those of the women in group A,and the levels of serum IL-4 and IL-10 of the women in group B were sig-nificantly lower than those of the women in group A(P<0.05).Point-biserial correlation analysis showed that the lev-els of IL-2,IFN-γ,TNF-α and IL-6 of the women with SCH were positively correlated with their adverse pregnancy outcomes,and the levels of IL-10 and IL-4 of the women with SCH were negatively correlated with their adverse preg-nancy outcomes(all P<0.001).Logistic regression analysis showed that the increased levels of serum IL-2 and TNF-α,IFN-γ,IL-6 and thyroid-stimulating hormone,and the decreased levels of IL-4 and IL-10 of the pregnant women with SCH were all the risk factors of their adverse pregnancy outcomes.Conclusion:The levels of IL-2,TNF-α,IFN-γ and IL-6 of the pregnant women with SCH are increased,while the levels of IL-4 and IL-10 are decreased,and all of which are correlated with the adverse pregnancy outcomes of the women.
